Mr. Ron Boone, of the NC Division of Water Resources (NCDWR), Winston-Salem Regional
Office (WSRO), conducted the subject compliance evaluation inspection, on February 23, 2022.
Mr. Boone met with Ken Wahl and Todd Robinson, both of Enviorlink Inc., who operate the
manor's wastewater treatment plant, as well as you and manor maintenance personnel. Inspection
findings are detailed in the attached Water Compliance Inspection Report and summarized below.
The manor was non -compliant with all requirements of the NC regulations that govern the proper
operation and maintenance of deemed permitted wastewater collection systems.
This office is issuing a Notice of Violation for this inspection. Please be aware that the Division
may take enforcement action against any permittee who fails to comply with any requirement of
their permit. Pursuant to NC General Statute (NCGS) 143-215.6A, a civil penalty of not more than
twenty-five thousand dollars ($25,000.00) may be assessed against any person who violates or
fails to act in accordance with the terms, conditions, or requirements of any permit issued pursuant
to G.S. 143-215.1.

Inspection Summary:
Ron Boone, of the NC Division of Water Resources (DWR), Winston-Salem Regional Office, met with Ron Wahl and Todd
Robinson, both of EnviroLink Inc, who are contracted to operate the manor's wastewater
treatment system, as well and Brittany White, manor administrator, and manor maintenance personnel on 2/23/22, to
perform a compliance evaluation inspection of the manor's wastewater collection system. The inspection findings are
summarized below and in the following checklist.
Although there were not any apparent problems or ongoing issues, deficiencies, or violations on the collection system
identified during the inspection, the manor is not compliant with any of the requirements they must meet under the NC
regulations that govern the operation and maintenance of deemed permitted wastewater collection systems as listed below:
(1) The collection system shall be effectively maintained and operated at all times to prevent discharge to land or surface
waters and to prevent any contravention of groundwater standards or surface water standards.
No records of operation and maintenance were available during the inspection.
(2) A map of the collection system shall have been developed and shall be maintained.
Original plans for the manor are available but their accuracy is in question. They're also quite old and have begun to
deteriorate. The manor should consider updating the maps and having them redrawn.
(3) An operation and maintenance plan, including pump station inspection frequency, preventative maintenance schedule,
spare parts inventory, and overflow response shall have been developed and implemented.
There is no written O&M plan.
(4) Pump stations that are not connected to a telemetry system shall be inspected by the permittee or its representative
every day, 365 days per year, unless the permittee demonstrates that daily inspections are not necessary because the
pump station has sufficient storage capacity, above the elevation at which the pump activates, to justify a longer inspection
interval. In no case shall the inspection interval exceed seven days. Pump stations that are connected to a telemetry
system shall be inspected once per week.
There are pump stations existing onsite that should be documented and operated and maintained as required.
(5) High -priority sewers shall be inspected by the permittee or its representative once every six -months, and inspections
shall be documented.
It is unknown whether any high priority sewers. The system must be evaluated for their presence and any found must be
documented and properly operated and maintained as required.
(6) A general observation by the permittee or its representative of the entire collection system shall be conducted once per
year.
No such observations have been recorded.
(7) Overflows and bypasses shall be reported to the appropriate Division regional office in accordance with 15A NCAC 02B
.0506(a), and public notice shall be provided as required by G.S. 143-215.1C.
No such overflows have been recorded or reported.
(8) A Grease Control Program shall be in place. For privately owned collection systems, the Grease Control Program shall
include bi-annual distribution of grease education materials to users of the collection system by the permittee or its

representative. Grease education materials shall be distributed more often if required to prevent grease -related sanitary
sewer overflows.
There is no such grease control program established or implemented.
(9) Right-of-ways and easements shall be maintained in the full easement width for personnel and equipment accessibility.
Right-of-ways have not been identified and therefore proper maintenance of them cannot be determined at this point.
(10) Documentation of all these requirements shall be maintained by the collection system owner for three years with the
exception of the map, which shall be maintained for the life of the system.
The collection system owner does not have any documentation from either the past operators or from Envirolink personnel,
who, technically, are not contracted to operate the collection system at this time. Failure of the owner to obtain copies of all
operations and maintenance records demonstrates why the past documentation is unavailable and the owner is in violation
of documentaion requirements.
