All — I've made most of the updates for this permit this morning. Attached is the automated alert that went to our billing
specialist this morning — she is the only one who can update the billing contact. She will do that as soon as she can (but
it may not be today).
Thank you so much for your time and patience as we worked through all the details. I especially appreciate the very
thorough emails and explanations of what should and should not change, and the verification with the NCSOS
record. These clear details were very helpful in a time of administrative support gaps in our office!

Hello again. This saga continues as I noticed the name on the COC and all information the State has is incorrect on the
name of the facility and owner.
On the COC and permit verification the name is listed as: Fibercrete Preservation Technologies, Inc.
If you look closely at the first word it is misspelled. Maybe spell check or the system or input or an original application
was incorrect, who knows but it does not match information from NC SOS. See attached.
Correct spelling is: FIBRECRETE PRESERVATION TECHNOLOGIES, INC.
Note it is Fibre not Fiber...

Will this require a new Permit Name/Ownership Change form or can it just be considered a misspelling and
be changed in the database?

I will have to research this further. I will consult a staff in DWR who does these to get some guidance on whether this is
acceptable.
Does this mean that the Parent Company/Permittee would stay the same name (Fibrecrete Preservation

I apologize for the confusion. Our Administrative Assistant Suzanne left us in October, and we only just hired a new
one. It took me some time to track down what happened here. It seems we processed the Owner Affiliation change
back in August, but the Name/Ownership change request was not accepted. Please see the attached email. The reason
was that " FPT Infrastructure is not a legal entity registered with the North Carolina Secretary of State (NCSOS).
Owner entity must be registered with the NCSOS."
Did anyone ever follow up with Suzanne McCoy about this? It is something she would check when she got requests for
changes to a permittee entity name.
We have a new Permit Name/Ownership Change form that you need to fill out if the company/permittee name still
needs to be revised. Please let me know if you have any questions about addressing the discrepancy with the NCSOS,
and proceed with submitting that request. I understand there are some updates that need to be made for Bart Pharr, so
you'll have an opportunity to let us know about those on that form.

change form were submitted in August 2021. We are curious why that information has not been updated. That is also
why we were requesting the updated COC to reflect these changes.

Please see the attached record of the Owner Affiliation change attached, and the permit summary report that reflects
current information in our system. The COC was renewed in 2019, and the attached letter was sent out about

that. However, we do not see a record that the renewal COC was ever requested (so it was not created for our
repository). Regardless, the COC permit is active (status is noted on the Permit Summary report). The next NCG160000
General Permit renewal is in 2024.
