The hard copy should be sent to our office (1612 Mail Service Center address below). We don't have a full time staff
person working on these right now, so we are very behind on registrations. Please keep uploading copies of DMRs to
our on-line form and mailing them in to the Regional office until you are notified about eDMR registration.
The first order of business is to get your owner information in order. Here is info on how to request updates (also on our
website):
How to Update Permit Data
Enter your stormwater permit number into the Stormwater Permit Summary Report and you will see a
form that includes the address of the facility, contact information for the permit, and the regulated
stormwater outfall(s) for the facility.
To update the:
• Facility/Company name or ownership, complete the Name/Ownership Change Form.
• Legally responsible person (Owner Affiliation), complete the Permit Contact Update Request Form.
• Delegation of Signature Authority (DOSA), complete the Permit Contact Update Request Form.
• Billing contact, complete the Permit Contact Update Request Form.
• Permit contact, complete the Permit Contact Update Request Form.
• Facility contact, complete the Permit Contact Update Request Form.
• Facility address only, email Bethany Georgoulias.
• Stormwater outfall information, email Bethany Georgoulias.

Good afternoon,
I need to get a few items updated could you help me with that. Also, I submitted the
eReporting Registration Form August of 2021 but the website has us checked no for Registered
for eDMR.
Please let me know if I need to do anything else, do I need to submit the hardcopies to you?
