Dear Mr. Knox:
Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant
to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between North Carolina and the U.S. Environmental Protection Agency dated May 9, 1994 (or as
subsequently amended).
The terms included in this final permit exhibit some changes from those found in draft permit sent to
you on November 17, 2009. These changes include:
• The reference to dechlorination in the facility description has been removed.
• Monitoring for Total Zinc has been removed because the facility does not use a zinc compbund as a
corrosion inhibitor. Monitoring for ammonia nitrogen has been removed because the WTP does not
use chloramines.
• Monitoring for Chronic Toxicity has been changed to specify monitoring during the months of
January, April, July and October. This monitoring schedule coincides with that for the wastewater
treatment plant. It is hoped this change will help in coordinating such testing with your laboratory.
While we are indeed sensitive to the fiscal and operational matters you pointed out in your November
30, 2009 letter, we are unable to make any further modification to the terms of the final permit. Its
terms are consistent with those included in NPDES permits for conventional WTPs throughout the state.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty
(30) days following receipt of this letter. This request must be in the form of a written petition,
conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of
Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless
such demand is made, this decision shall be final and binding.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby
revoked. As of this permit issuance, any previously issued permit bearing this number is no longer
effective. Therefore, the exclusive authority to operate and discharge from this facility arises under the
permit conditions, requirements, terms, and provisions included herein.
The Town of Pilot Mountain

1. Continue to operate an existing water treatment plant filter backwash system consisting of the
following components:
• Gravity thickener with plate settlers
• Eight sand drying beds

1. The Division shall consider all effluent TRC values reported below 50 µg`/L to be in compliance
with the permit. However, the Permittee shall continue to record and submit all values reported by
a North Carolina certified laboratory (including field certified), even if these values fall below 50
µme•
2. The pH shall not be less than 6.0 standard units, nor greater than 9.0 standard units.
3. Monitoring should be performed in conjunction with toxicity testing.
4. Toxicity testing is to be conducted during the months of January, April, July and October. See
Condition A. (2.) for toxicity testing requirements. Toxicity sampling should coincide with
sampling for parameters covered by footnote 3.
There shall be no floating solids or foam in other than trace amounts.

A. (2.) CHRONIC TOXICITY MONITORING (QUARTERLY)
The permittee shall conduct quarterly chronic toxicity tests using test procedures outlined in the "North
Carolina Ceriodaphnia Chronic Effluent Bioassay Procedure," Revised February 1998, or subsequent
versions.
The effluent concentration defined as treatment two in the procedure document is 3 %. The testing
shall be performed as a Ceriodaphnia dubia 7day pass/fail test. The tests will be performed during
the months of January, April, July and October. Effluent sampling for this testing shall be
performed at the NPDES permitted final effluent discharge below all treatment processes.
All toxicity testing results required as part of this permit condition will be entered on the Effluent
Discharge Monitoring Form (MR-1) for the month in which it was performed, using the parameter
code TGP3B. Additionally, DWQ Form AT-1 (original) is to be sent to the following address:

Completed Aquatic Toxicity Test Forms shall be filed with the Environmental Sciences Section no
later than 30 days after the end of the reporting period for which the report is made.
Test data shall be complete and accurate and include all supporting chemical/physical measurements
performed in association with the toxicity tests, as well as all dose/response data. Total residual
chlorine of the effluent toxicity sample must be measured and reported if chlorine is employed for
disinfection of the waste stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is
required, the permittee will complete the information located at the top of the aquatic toxicity (AT) test
form indicating the facility name, permit number, pipe number, county, and the month/year of the
report with the notation of "No Flow", in the comment area of the form. The report shall be submitted
to the Environmental Sciences Section at the address cited above.
Should the permittee fail to monitor during a month in which toxicity monitoring is required, then
monthly monitoring will begin immediately. Upon submission of a valid test, this monthly test
requirement will revert to quarterly in the months specified above.
Should any test data from this monitoring requirement or tests performed by the North Carolina
Division of Water Quality indicate potential impacts to the receiving stream, this permit may be re-
opened and modified to include alternate monitoring requirements or limits.
If the Permittee monitors any pollutant more frequently then required by this permit, the results of such
monitoring shall be included in the calculation & reporting of the data submitted on the DMR & all AT
Forms submitted.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum control
organism survival, minimum control organism reproduction, and appropriate environmental controls,
shall constitute an invalid test and will require immediate follow-up testing to be completed no later
than the last day of the month following the month of the initial monitoring.

Thanks for the reply Bob. We use only chlorine as a disinfectant and Sodium Hexametaphosphat
for corrosion control. Our water system operates at a deficet and any reduction in
monitoring will be greatly appreciated in the final permit. We do not have a effluent flow
meter the reported values in the past were influent amounts. As stated before this system
was designed to treat about 10 times the alum sludge as it does now. The effluent valve is
only opened monthly to keep it exercisrd and samples are taken from within the lagoon itself.
My cell # IS 336-648-4156 the plant # 336-368-4592 is connected to my SCADA modem for remote
access.

Dear Mr. Bob Sledge,
We are in receipt of our draft permit NC0068365 and would like to respectfully
make the following comments :
1. The alum sludge discharge line does not have a flow meter installed on it as the draft
permit states.
2. The alum sludge lagoon does not have dechloriniation.
3. The expanded testing proposed in this permit ( toxicity and other chemical
compounds) will greatly impact the operation of this facility financially.
Pilot Mountain has the second highest water rate schedule of all municipalities in Surry
County and we are laboring under a tremendous burden of debt. In exploring all of our options
such as abandoning this discharge and hauling the alum sludge to our waste water treatment
plant and/or seeking a no return option of pumping the effluent to our raw water reservoir there
seems to be no economical solution.
Our water treatment plant which is designed to treat 1,500,000 gallons daily is currently
treating approximately 1,500,000 weekly. Eighty percent of the water going to the alum
sludge lagoon is (filter to waste water) with little or no residual chlorine in it. Our plant only
operates approximately 8 out of 48 hours and this filter to waste is approximately 10,000
gallons of those 48 hours.
The vast majority of the contents of the alum sludge lagoon is evaporated out due to
extended holding times. The actual discharge valve is only operated monthly to keep it
exercised.
It is our opinion that the options that are described above and the costs associated
with them will require a substantial water rate increase to our citizens and would not greatly
increase the impact of the water quality of Toms Creek.
We would respectfully request that our current limitation criteria in our current permit
be continued as the Town of Pilot Mountain is barely financially solvent.
Thanks again for your consideration in this matter and we are hopeful that our dire
situation will affect your decision.

Sludge Disposal
Our water treatment plant has seen our usage drop drastically due to the
closing of our textile industry. We have a water treatment capacity of 1.5 MGD
but we are only treating about 1.5 million per week. This means there is very little
alum sludge produced. We have not removed the alum sludge residue from
our settling pond in several years but we took the sludge to the land fill
and would propose to do so when needed in the future.
