Dear Mr. Hogan:
In accordance with the application for discharge permit received on October 30, 1998, the
Division is forwarding herewith the subject NPDES permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between North Carolina and the U.S. Environmental Protection Agency dated December 6, 1983.
The conditions in this final permit include one change from the draft permit we sent to you in
November 1998.
• The name of the facility was corrected to The Patterson School, Inc.
If any parts, measurement frequencies, or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within
thirty (30) days following receipt of this letter. This request must be in the form of a written
petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the
office of Administrative Hearings, Post Office Drawer 27447, Raleigh, North Carolina 27611-
7447. Unless such a demand is made, this permit shall be final and binding.
Please take notice that this permit is not transferable except after notice to the Division of Water
Quality. Part II, E.4. addresses the requirements to be followed in case of change in ownership
or control of this discharge. The Division of Water Quality may require modification or
revocation and reissuance of the permit.

Summary

The Total Suspended Residue levels in August were apparently caused by
algae growing in the WWTP. Our plant is lightly loaded even when school
is in operation, and after our small summer school ended in July there
was very law water usage, with no kitchen activity and only a few residences
occupied. The summer was unusually dry and sunny, and flow through the
plant at a minimum, and this apparently led to algae growth.
I had not experienced algae in the plant before, so did not recognize the
problem it could cause. After taking the third effluent sample in Aug.
and finding the TSS even higher than the first two had been I asked our
lab what I should do to improve the TSS. After I mentioned the green
color• I had noticed in the samples they said that algae could be the cause,
and a weak copper sulfate -treatment would reduce the algae. I started
maintaining a 1 PPM level in the plant in early September.
The first Sept. sample was way below the last August sample, but still too
high. The second was even lower but still above the limit (for the month) .
The third sample was way below either of them, and it pulled the average
down to 27.5, making it in compliance for Sept. Oct. samples have all been
well under the level of concern. I stopped the copper sulfate treatment
in mid 0ct ober .
I apologise for this situation which I should have caught in time to prevent
our being out of compliance for the month. I believe I took the correct
action once I was aware there was a problem.

CHRONOLOGY AND NOTES
<10/6/98> <The last WLA done for the school was done in 1989 for relocation to the Yadkin
River. The Morgan School had been discharging to Barnett Branch and had
requested relocation to the larger Yadkin River. The relocation was approved by
DWQ. Recommended limits were for secondary treatment (30/30). Facility was
streamlined in 1993. Verification of the relocation was received via telephone call
on 10/6/98 to the ORC, Mr. Jim Hogan. He indicated that the relocation had been
done several years ago, approximately 1000 ft of pipe had been laid and that Mr.
Carroll Williams, an engineer from Hickory had done plans and pipe was laid to
specifications. Mr. Hogan said that they might not have sent in info that told us
that the project had been completed.
<10/6/98> <Review of DMRs for 1996-1997. (IMS system is not printing out compliance
data.) The monthly average limits have been met consistently for the past two
years and the plant appears to operating well. There is one instance of a daily
value for TSS of 41 mg/1, however this did not exceed the daily maximum limit of
45 mg/1.
Recommend the renewal of existing limits and re -issuance of the permit.
