I apologize for the confusion. Our Administrative Assistant Suzanne left us in October, and we only just hired a new
one. It took me some time to track down what happened here. It seems we processed the Owner Affiliation change
back in August, but the Name/Ownership change request was not accepted. Please see the attached email. The reason
was that " FPT Infrastructure is not a legal entity registered with the North Carolina Secretary of State (NCSOS).
Owner entity must be registered with the NCSOS."
Did anyone ever follow up with Suzanne McCoy about this? It is something she would check when she got requests for
changes to a permittee entity name.
We have a new Permit Name/Ownership Change form that you need to fill out if the company/permittee name still
needs to be revised. Please let me know if you have any questions about addressing the discrepancy with the NCSOS,
and proceed with submitting that request. I understand there are some updates that need to be made for Bart Pharr, so
you'll have an opportunity to let us know about those on that form.

Yes, we received the email, thank you. Please note that both a Name/Ownership change form and Owner Affiliation
change form were submitted in August 2021. We are curious why that information has not been updated. That is also
why we were requesting the updated COC to reflect these changes.

I accessed the original system to generate the COC as it would have looked in 2019. 1 hope you received the email, but it
is attached for reference.
The current info in our system still has the name on the COC: Fibercrete Preservation Technologies, Inc. If there has
been a company name change since then, please use our Permit Name/Ownership Change form to submit the necessary
paperwork.

Please see the attached record of the Owner Affiliation change attached, and the permit summary report that reflects
current information in our system. The COC was renewed in 2019, and the attached letter was sent out about
that. However, we do not see a record that the renewal COC was ever requested (so it was not created for our
repository). Regardless, the COC permit is active (status is noted on the Permit Summary report). The next NCG160000
General Permit renewal is in 2024.
