Hi, Kenny,
I will follow up with you more tomorrow. We did get the Owner Affiliation change, and that was processed, but the I
don't think the Name/Ownership change was accepted. That email notification went out on 8/26/2021, but it wasn't
imported into the permit file, so I had to track it down in my CC's. I had another email from James Willard about this
today, and I'll copy all of you with some more details tomorrow when I can pull them together.
I apologize for the confusion. We will need to direct you to the new form we use now, but I'll include instructions for all
that in my next correspondence. We have been without a Program Admin Assistant since October, so we are also
behind on many other requests that came in since then. We are only just training up a new one this week, so please be
patient with us as we work to get it all sorted out.
Thanks,
Bethany Georgoulias (she/her)

appointments only. Please check with the appropriate staff before visiting our offices, as we may be able to handle your

Hi Bethany,
I left you a voicemail regarding the COC and Owner Affiliation Change Form to inquire about its status.
An Owner Affiliation Change Form was submitted in August of 2021 and a NPDES Stormwater (2021) Renewal
COC Request was submitted on 3-4-2022. There has been a job title and email change for Bart Pharr since the
August 2021 form submittal. Please let me know how to proceed. Thank you.

Good afternoon,
I accessed the original system to generate the COC as it would have looked in 2019. 1 hope you received the email, but it
is attached for reference.
The current info in our system still has the name on the COC: Fibercrete Preservation Technologies, Inc. If there has
been a company name change since then, please use our Permit Name/Ownership Change form to submit the necessary
paperwork.
Best regards,

Bethany,
Thank you for the information. Yes, we noticed that the COC was not in the repository and therefore made the COC
request last week. Please see attached. We do need an updated COC issued as we have requested name and ownership
affiliation change as you noted. Thank you for your time and assistance with this. Have a blessed day.

Good morning,
Please see the attached record of the Owner Affiliation change attached, and the permit summary report that reflects
current information in our system. The COC was renewed in 2019, and the attached letter was sent out about
that. However, we do not see a record that the renewal COC was ever requested (so it was not created for our
repository). Regardless, the COC permit is active (status is noted on the Permit Summary report). The next NCG160000
General Permit renewal is in 2024.
Please let us know if you have any other questions.
