Duke Energy requests that Part VI.4 of the permit be modified to submit the Injection Event Report annually in lieu of 30
days after each injection. I understand that the requirement of submittal of Injection Event Records (IERs) 30 days after
each injection may not be suitable to the nature of injection at the Belews Creek site. However, without any data such as
pilot test data, there is no basis for me to determine whether the requested modification is reasonable. If similar or
same modifications requested for Allen and Marshall have already approved, to keep consistent, I would recommend

the same. Otherwise, I think we can ask for the first year, submit IERs quarterly, then for the following years annually if
not hear different from us. In this case, we do have opportunity to find problems within a relative short time. So I would
like to discuss with you and Eric if necessary before draft a Staff Report. I will be available about anytime. For
evaluation of the requested modification, I do not think a site visit is necessary, so we should provide you a staff report
quickly.

Just wondering where you all were on a staff report for this permit modification request. It will be going statutory next
month. If there is not enough time/staff to conduct a site inspection, a suggestion would be to review the application,
talk to Permittee/consultant on the telephone (if needed), and then do the Staff Report. Later, if warranted, conduct a
site inspection.

Hello -
Attached is a permit modification request application for Duke's Belews Creek Facility. The permit modification request
is for the reporting requirements for submittal of Injection Event Records (IERs) to be changed from 30 days after each
injection to annually. These IERs are really designed for groundwater remediation injection wells where there are a
limited number of individual injections of remediation additives. The IERs are not for the type of system that Duke has
in which large volumes of water are injected 24/7. However, the injection data will still be reported, but will be included
in an annual report. If you agree, the permits will be re -issued with this modification.
