Subject: Revisions and Valid Signatures Requested - Juniper PS FastTrack Application
Good morning Mr. Grey:
The application for the Juniper Pump Station Renovation and WWTP Demolition has been received in our office. I have
looked over it, and it appears that I will need a few revisions before proceeding:
• For each of the FTSE 10-18 forms included (both from Mr. Lambert and from Ms. Driver), we will need either
original wet -ink signatures, or a secure digital signature applied by a program such as DocuSign or Adobe.
Unfortunately, photocopies of signatures and typed signatures can't be accepted.
• On page two of form FTA 06-21, please check the appropriate boxes for items VI.1, VI.2, VI.3, and VI.6 (I assume
these were left blank as they are not applicable to the project or "no", but they all must be filled out for the
form to be considered complete).
• Also on page two of form FTA 06-21, please revise items VI.7 and VI.8 to reflect the requested allocation of
250,000 GPD as listed on the FTSE forms. The table does not need to be thoroughly complete, but the total
should be entered in the bottom right hand corner. Item VI.8 currently lists 152,000 GPD, which I imagine is a
typo.
Feel free to send the FTA 06-21 edits in PDF form to avoid the delays of mailing in a hard copy. I will simply pull out the
edited page and file it with the original copy.
