4. Project Description and History
4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:*
The 108 acre Trivium Corporate Center East property is currently vacant consisting of pasture and wooded areas. Surrounding land uses to the west consist of industrial development,
residential development to the north and immediately to the west.
This project has two stream crossings associated with the proposed sewer line extension. The first stream crossing is required for general replacement of the sewers across Clark Creek
as maintenance. The contractor will not be allowed to cross the creek with equipment or install a temporary crossing. The second crossing will across a tributary to Clark Creek in an
existing sewer right-of-way. A temporary creek crossing will be utilized for equipment/material access and removed after construction is complete. Both crossings will be restored with
native vegetation.
4b. Have Corps permits or DWR certifications been obtained for this project (including all prior phases) in the past? *
Yes No Unknown
4f. List the total estimated acreage of all existing wetlands on the property:
4g. List the total estimated linear feet of all existing streams on the property:
4h. Explain the purpose of the proposed project: *
The proposed project will consist of the construction of approximately 2600 linear feet of 8 and 12-inch diameter gravity sewers to serve industrial and residential development in the
vicinity.
4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used: *
The City of Hickory plans to construct approximately 2600 linear feet of 8 and 12-inch diameter gravity sewers to serve industrial and residential development in the vicinity. The project will
begin at the existing 12-inch diameter gravity sewers along the tributary to Clark Creek and extend westward. It is anticipated that two track hoes, and a rubber tired loader will be utilized
during construction.
At this time no other direct and indirect impacts to environmental resources are anticipated at this time.
Erosion control measures will include temporary creek crossing with rock backfill (to be removed after construction), temporary silt fence, silt fence outlets and permanent seeding.
5. Jurisdictional Determinations
5a. Have the wetlands or streams been delineated on the property or proposed impact areas?*
Yes

E. Impact Justification and Mitigation
1. Avoidance and Minimization
la. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project:
The impact widths are reduced to the minimum required to install the proposed sewers.
lb. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:
Routing of proposed sewer line generally follows the tributary to Clark Creek - a minimum of 30' undisturbed buffer will be maintained parallel to the
water features. Widths of the Temporary Impacts will be minimized as much as possible. The new crossing will be installed within an existing City of
Hickory force main easement to limit the overall width of disturbance.
2. Compensatory Mitigation for Impacts to Waters of the U.S. or Waters of the State
2a. Does the project require Compensatory Mitigation for impacts to Waters of the U.S. or Waters of the State?
Yes No
2b. If this project DOES NOT require Compensatory Mitigation, explain why:
Project does not require mitigation due to temporary impacts
F. Stormwater Management and Diffuse Flow Plan (required by DWR)
1. Diffuse Flow Plan
la. Does the project include or is it adjacent to protected riparian buffers identified within one of the NC Riparian Buffer Protection Rules?
Yes No
If no, explain why:
No changes in the stormwater flow patterns are proposed in the installation of the gravity sewers.
2. Stormwater Management Plan
2a. Is this a NCDOT project subject to compliance with NCDOT's Individual NPDES permit NCS000250? *
Yes No
2b. Does this project meet the requirements for low density projects as defined in 15A NCAC 02H .1003(2)?
Yes No
Comments:
This project is not part of a development - utility installation only with no proposed impervious areas as part of the project.
G. Supplementary Information
1. Environmental Documentation
la. Does the project involve an expenditure of public (federal/state/local) funds or the use of public (federal/state) land? *
Yes No
1 b. If you answered "yes" to the above, does the project require preparation of an environmental document pursuant to the requirements of the National or State (North Carolina)
Environmental Policy Act (NEPA/SEPA)?*
