The North Cazolina Deparhnent of Transportation requests authorization to replace a
deterioraring 60-foot, 36" CMP with a 60-fooY, 42" CMP. The existing pipe is perched
approximately 6 feet and is �3.5% slope. A 15-ft long scourhole has formed at the outlet. To
bury the pipe to meet permit condiTions would result in a steeper slope of; therefore, it is proposed
to plaw the pipe inlet at streambed elevation and leave the new pipe perched, with The
construction of a"rock ladder" To provide limited aquatic passage. The remaining portion of the
scourhole would be annored to provide energy dissipation. Due to the site conditions at the
project site, NCDOT requests a waiver from the Regional Co�ditions 2(b)(e) permit depth
speci£cafion far the wnstrucflon of this project. Temporary cofferdam pumping systems will
be used to divert the streams around the construction sites as needed. Any areas affected by the
temporary cofferdun will be restored to pre-project conditions. The scope of the work will result
in 20 feet of permanent impacts to an unnamed fributary to Hildebran Creek, which is classified
as C1ass C Waters by the NC Division of Water Resources.(15 feet of rock ladder/scourhole
armoring at outlet+ 5 feet of riprap bank impacts at inlet).
The project is not located within '/z mile of any critical watershed area nor is SR 1153 a rural or
urban arterial; therefore, hazardous spill basins are noT required. Enclosed is the hazardous spill
basin checklist.
The SYaYe Historic Preservation office resources and The Nat�ional Register of Historic Places were
reviewed for historic sites in the vicinity of the project. There are no known sites and proposed
impacts are witliin previously disturbed NCDOT right of way. This office has conducted a GIS
and field survey for the project threatened/endangered species on April 7, 2014. The federally
listed species for Caiawba County (USFWS listed date September 22, 2070) include dwarf-
flowered heartleaf (Hexastylis nan�ora) and schweiniYz's sunflower (Helianthus schweinitzii).

of the proposed projecf area. The heartleaf is usually found on dxy to moist ravine slopes
(usually north facing and of the oak-hickory-pine forest type) and in boggy areas near
springheads. There is no suitable habitat in the proposed project area and no heartleaf
species was observed within the project footprint. Natural Heritage Program records
document the nearest Helianthus Schweinitzii location approximately 14.2 miles east of
the proposed project area. Although the survey was conducted outside of the opfvnal
survey window, no plants were observed and the site is located in a xegularly maintained
NCDOT right of way in a residential neighborhood; therefore, no impact on the species is
anticipaied.

The purpose of the proposed work is to replace the deteriorating culvert for the safety of the travelling public.

The North Carolina Department of Transportation requests authorization to replace a deteriorating pipe on SR 1153, Long
Avenue in Catawba County.
Replace the existing 60-foot, 36" CMP with a 60-foot, 42" CMP. The existing pipe is perched approximately 6 feet and is
-3.5°/o slope. A-15 ft long scourhole has formed at the outlet. To bury the pipe to meet permit conditions would result
in a steeper slope of; therefore, it is proposed to place the pipe inlet at streambed elevation and leave the new pipe
perched, with the construction of a"rock ladder" to provide limited aquatic passage. Large riprap will be embedded into
the remain portion of the existing scourhole to provide energy dissipation.
Temporary cofferdam pumping system will be used to divert the streams around the construction site as needed. All
areas affected by temporary cofferdam system will be restored to pre-project conditions. The Project will be constructed
using typical heavy equipment (trackhoe, backhoe, etc.)
