In accordance with the application for discharge permit received on February 20, 1998, the Division is
forwarding herewith the subject state - NPDES permit. This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and
the U.S. Environmental Protection Agency dated December 6, 1983.
If any parts. measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days
following receipt of this letter. This request must be in the form of a written petition, conforming to Chapter
150B of the North Carolina General Statutes. and filed with the office of Administrative Hearings, Post Office
Drawer 27447, Raleigh. North Carolina 27611-7447. Unless such a demand is made. this permit shall be
final and binding.
This permit does not affect the legal requirements to obtain other permits which may be required by
the Division of Water Quality or permits required by the Division of Land Resources, Coastal Area
Management Act, or any other Federal or Local governmental permits which may be required.

Conductivity monitoring (U&D) is still shown on the Effluent
Limitations and Monitoring Requirements page. Comments in the
Instream Monitoring Requirements of the Fact Sheet indicated that
conductivity was going to be dropped. WSRO concurs with dropping.
Do you need anything else back from WSRO? There were no signature

Background
This will be a discharge of backwash water from a conventional alum water
treatment. The facility was originally permitted in 1997 and has not yet been
constructed.
Wasteload Allocation Summary
This facility was most recently permitted in accordance with the Division's guidance
for water treatment plant backwash discharges dated September 30, 1992.

This facility does not have a compliance history as operation has not begun.

The Division does not currently require toxicity testing for conventional water
treatment plant backwash discharges.
Proposed Changes
A flow limit was installed in the original permit. The Division does not currently
limit flow from water treatment plant discharges. This flow limit will therefore be
dropped. Additionally, upstream and downstream turbidity monitoring was required
but no locations were specified. These locations will be specified with this renewal.
Summary and Recommendations
It is recommended that this permit be renewed in accordance with the Division's
guidance for water treatment plant backwash discharges.

The Division is beginning the second five-year cycle of river basin planning and permit renewals. An
examination of the basin planning schedule has revealed that the timing of permit renewals does not
allow an evenly distributed workload as first envisioned. This results in problems with the efficiency
and effectiveness of the NPDES program. In an effort improve customer service, and after thorough
review of the water quality issues in your area, the Division is changing the permit renewal schedule for
all NPDES permits in your sub -basin.
This permit modification changes the expiration date of the subject permit to May 31, 1999. Please find
enclosed the revised permit cover page. Insert the new cover page into your permit and discard the old
page. All other terms and conditions contained in the original permit remain unchanged and in full
effect. This permit modification is issued pursuant to the requirements of North Carolina General
Statutes 143-215.1 and the Memorandum of Agreement between North Carolina and the U. S.
Environmental Protection Agency.
If you have already submitted a permit renewal application for the subject facility, the Division will
retain the application until your permit is due for renewal. No additional forms or fees will be required.
If any parts, measurement frequencies or sampling requirements contained in this permit modification
are unacceptable to you, you have the right to an adjudicatory hearing upon written request within
thirty (30) days following receipt of this letter. This request must be a written petition conforming to
Chapter 150B of the North Carolina General Statutes, filed with the Office of Administrative Hearings,
Post Office Drawer 27447, Raleigh, North Carolina 27611-7447. Unless such demand is made, this
decision shall be final and binding.

Dear Permittee:
After a thorough review of the Yadkin -Pee Dee River basin management plan and water quality issues relevant to
the subject facility, the Division of Water Quality has decided to extend the duration of the subject permit. The
expiration date of your permit will be changed in order to allow the Division to review all NPDES discharge
permits in the Yadkin River Basin during calendar year 1999.
All terms and conditions in the existing permit will apply until a new expiration date is assigned by the Division.
Extension of existing Effluent Limitations and Monitoring Requirements does not exempt the subject facility from
the new Enforcement Policy recently enacted by the Division.
Attached to this letter is a copy of the Public Notice that is being published to announce the planned extension of
your permit. After Public Notice has been made by the Division, the expiration date of your permit will be
changed via a permit modification. No draft permit will be sent out with the Public Notice, as none of the
conditions in the existing permit will be modified.
The Division appreciates your patience and understanding to date regarding the rescheduling of the subject
permit. If you have already submitted your renewal application package, no additional fees or documentation will
be required as a result of this change to your permit's expiration date.

industrial wastewater tom two outtalk into Yadkin River (001) and Mud-
dy Creek (002), CIassWS-IV streams in the Yadkin -Pee Dee River Basin.
The Yadkin Rorer has a 7010 flow of 554 its and Muddy Creek has a
7010 flaw of 55 cis. CBODS, ammonia and dissolved eaygen are water
quality limited. For some parameters, the available load capacity of the
immediate receiving wale will be consumed. This may affect furore wa-
ter quality based effluent limitations for additional dischargers within this
parson of the watershed. •

3. NPDES Permit NC0084212 Davie County (728 Sparks Road, Mock -
sae, NC 27028) has applied for renewal of the NPDES permit for the
Sparks Road Water Treatment Plant, located at 728 Sparks Road north-
east of Farmington in Davie County. The fealty discharges filter back-
wash from one outfall into the Yadkin River, a Class WS-N stream in the
Yadkin-Pee.Dee River Basin. Turbidity Is water quality limited. This may
affect future water quality based effluent limit tons for additional dis-
chargers within this portion of the watershed,
4. NPDES No. NC1086011.The City of Winston-Salem; P.O. Boa 2511,
Winston-Salem, NC 27102, has applied for renewal of its NPDES permit
for a fealty located at the Aobert Neilson Water Treatment Plant Frye
Bridge Road, Winston-Salem, Forsyth County. The facility discharges
treated filter backwash from one outfall into an unnamed tributary to
Muddy Creek, a Class C stream in the Yadkin -Pee Dee River Basin. No
parameters are water quality limited, but this discharge may affect future
