NPDES permit NC0079014 is hereby rescinded, effective immediately. This message is to close the
permit file for the Division, and for your records.
If in the future you wish to discharge wastewater to the State's surface waters, you must first
apply for and receive a new NPDES permit.
PLEASE NOTE: as part of the Division's conversion to paperless communication, this email is the
only notification you will be sent regarding the permit rescission. No hard copy will be mailed.

Apologies for the response delay to the permit rescission request. If Scott has already provided a response, please

I have reviewed the photos that Mr. Oula Shehab-Dandan provided in his Dec. 15, 2021 email. I conducted an inspection
of the facility on 1/24/2019, and recall the effluent discharge to surface water was eliminated with the reuse water
directed to the cooling tower, which eventually discharged to the local sanitary sewer system. As part of the rescission
request review, here's the Laserfiche link to that inspection report.
Given my review of the photos Mr. Oula Shehab-Dandan provided showing the possibility of discharge has been
removed and from my observations & findings during my inspection in 2019 confirming discharge elimination, this office
is comfortable with rescinding NPDES permit NC0079014.
