received December 30, 2021. However, additional information is required before the review may be

2022.

A. Operator of Responsible Charge:
1. Our records indicate that the certification status for Michael A. Ipock (Cert. no. 987554) is invalid.
Upon the Water Pollution Control System Operators Certification Commission's (WPCSOCC)
classification of the subject non -discharge facilities, the Permittee shall designate and employ a
certified operator in responsible charge (ORC), and one or more certified operators as back-up
ORCs. The ORC or their back-up shall operate and visit the facilities as required by the
WPCSOCC. [15A NCAC 02T .0117]
Please designate a back-up ORC for this facility by completing the ORC designation form
