4. Project Description and History
4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application: *
Piedmont Natural Gas was given previous authorization under a Nationwide Permit 12 (NWP12) to construct the Ramsey Road segment of the Highway 17 North Carolina Department of
Transporation (NCDOT) Distribution Replacement Project (Project) near the City of Jacksonville in Onslow County, North Carolina.
Based on review of a post -construction as -built survey for the Project, it was determined that a deviation from the original design occurred during construction. This deviation resulted in an
increase of total wetland impact acreage from what was previously authorized.

4h. Explain the purpose of the proposed project:
The Project was constructed as a replacement for an existing natural gas distribution pipeline that extends along Highway 17. This portion of pipeline was decommissioned and removed
due to a NCDOT project involving Highway 17. The NCDOT project consisted of widening and reconfiguring Highway 17, and the existing pipeline was relocated to allow for the expansion.
41. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:*
The Project consisted of installing approximately 16,922 linear feet of 8-inch medium -dense polyethylene (MDPE) natural gas distribution pipeline. Construction was utilized through open
trenching, horizontal directional drilling (HDD), and auger bore methods. The Project consisted of using heavy construction equipment (e.g., excavators, dozers, scrapers, side booms,
etc.). The Project is located along a 3.19-mile section of Ramsey Road (Route 1324) and Kellum Loop Road, northeast of the City of Jacksonville. The Project route starts approximately
0.17 miles east of the intersection of Route 1324 and Carolina Forest Boulevard, proceeds east along Route 1324 for approximately 3.12 miles, turns southeast and proceeds for 0.17
miles along
Kellum Loop Road, and ends at New Bern Highway (Highway 17). The Project spanned across wetlands and tributaries associated with Wolf Swamp. The majority of the construction
easement was located within existing maintained utility easements.

Project was originally authorized under a NWP12 for 1.05 acres of permanent wetland conversion impacts. Based on review of as -built survey, the
Project resulted in 1.19 acres of permanent wetland conversion impacts, which is an increase of 0.14-acre. See uploaded aquatic resource impacts table
in Miscellaneous section for wetland impacts. Permanent impacts are wetland conversion impacts from PSS to PEM for post -construction pipeline
maintenance. Permanent lost of waters did not occur as a result of the Project.

The Project was originally authorized under a NWP 12 for 87.06 linear feet (LF) of temporary impacts to jurisdictional (JD) ditches and 30.41 LF of
temporary impacts to stream channels. Based on review of the as -built survey, the Project resulted in 139.60 LF of impacts to JD ditches and 21.17 LF
of impacts to stream channels. This is an increase of 52.54 LF of JD ditch impacts and a reduction of -9.24 stream channel impacts. See uploaded
aquatic resource impacts table in Miscellaneous section for jurisdictional ditch and stream impacts.
E. Impact Justification and Mitigation
1. Avoidance and Minimization
1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project:
Avoidance and minimization of impacts to wetlands and other waterbodies was attempted by routing the pipeline within and adjacent to already existing
and maintained utility and NCDOT easements. The temporary work area and permanent maintenance easement were kept as narrow as possible to
minimize disturbances during construction and maintenance.
1 b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:
The majority of impacts were temporary, and the Project did not result in a loss of wetlands or other waterbodies. The pipeline was partially installed
using HDD, which means the permanent pipeline easement did not require earth work. Construction matting was utilized within wetland areas to
minimize disturbance. An approved and certified erosion and sediment control plan was provided to the contractor for proper implementation of BMPs.
