(J Yes Li No If no please,provide a written description why the facility was not compliant,the dates,and explain corrective action taken. 1) Only residuals approved for this permit were applied to the permitted sites. 2) Soil pH was adjusted as specified in the permit and lime was applied(if needed)to achieve a soil p1-1 of at least 6.0 or the limit specified in the permit. 3) Annual soils analysis were performed on each site receiving residuals during the past calendar year and three(3)copies of laboratory results are attached. 4) Annual TCI.P analysis(if required)was performed and three(3)copies of certified laboratory results are attached. 5) All other monitoring was performed in accordance with the permit and reported during the year as required and three(3) copies of certified laboratory results are attached. 6)The facility did not exceed any of the Pollutant Concentration Limits in 15A NCAC 02T.1105(a)or the Pollutant Loading Rates in 15A NCAC 02T.1105(b)(applicable to 40 CFR Part 503 regulated facilities). 7)All general requirements in as specified in the Land Application Permit were complied with(applicable to 40 CFR Part 503 regulated facilities). 8)All monitoring and reporting requirements in 15A NCAC 02'1'.1111 were complied with(applicable to 40 CFR Part 503 regulated facilities). 9)All operations and maintenance requirements in the permit were complied with or,in the case of a deviation, prior authorization was received from the Division of Water Quality. 10) No contravention of Ground Water Quality Standards occurred at a monitoring well or explanations of violations are attached to include appropriate actions and remediations. 11) Vegetative cover was maintained and proper crop management was performed on each site receiving residuals,as specified in the permit. 12) No runoff of residuals from the application sites onto adjacent property or nearby surface waters has occurred. 13) All buffer requirements as specified on the permit were maintained during each application of residuals. (CFIH), S&ME, Inc. (S&ME) is submitting the calendar year 2021 annual groundwater monitoring report for the CFN site located in Cofield, North Carolina (Figure 1) as required by the Land Application Permit (Permit No. WQ0012404, effective January 1, 2021). S&ME completed this annual report while under contract with CFIH. Groundwater Recovery and Distribution Volumes Based on weekly flow meter readings recorded from Transfer Pump Station #1 beginning December 24, 2020, and ending December 31, 2021, approximately 15,554,600 gallons of groundwater were removed from the confined shallow aquifer in 2021. Quarterly flow amounts for 2021 Transfer Pump Station #1 were as follows: 1st Quarter(January— March 2021): 3,020,600 gallons 2nd Quarter(April —June 2021): 4,048,600 gallons 3rd Quarter(July— September 2021): 3,434,700 gallons 4th Quarter(October— December 2021): 5,050,700 gallons Total Groundwater Recovery for 2021: 15,554,600 gallons Based on flow meter readings from Transfer Pump Station #2, approximately 14,795,000 gallons of water were transferred from the storage basin for subsequent land application. The nitrogen-enriched groundwater was applied to a variety of crops throughout the year including wheat, soybeans, and fescue. The following volumes of groundwater may differ slightly to those reported in the land application report, because meter readings were recorded on dates when weekly inspections were conducted, which may have been different than the days readings were recorded by the land application contractor. Groundwater pumping for land application efforts for 2021, on average, maintained the groundwater elevation within the slurry wall below 30 feet mean sea level (ft-msl).There were transitory exceptions early in the year followed by improved groundwater elevations. Onsite groundwater elevations were measured quarterly during 2021 (January, April,July, and October). The groundwater elevations recorded from October 27, 2020, through January 25, 2022, are presented in Table 1. As required in the permit, potentiometric maps showing quarterly interpretations of general groundwater elevations based on end-of-month measurements for January, April,July, and October 2021 are included as Figures 2, 3, 4 and 5. As required in Attachment C of the Non-Discharge Permit, cross-sectional views of water levels measured in piezometers PI-11 — P0-12 and PI-2— P0-236 were prepared and are attached as Figures 2A, 3A, 4A and 5A for each corresponding quarterly potentiometric map. Additional cross-sectional views of water levels measured in C- designated wells C-23—C-22, C-29—C-28 are included as Figures 2B, 3B, 4B and 5B, and cross-sectional views of C-33-C-32 and monitor wells MWI-4— MWI-6 are depicted in Figures 2C, 3C, 4C and 5C. The history of water levels for piezometers PI-2, PI-11, P0-12 and P0-236 is graphically shown in Figure 6. The historical water level data from the oldest C-designated wells on site (C-22, C-23, C-28, C-29, C-32 and C-33) are graphically shown in Figure 7. Historical water level elevation data for MWI-3, MWI-4 and MWI-6 are depicted in Figure 8. Precipitation events were recorded by an on-site rain gauge. The monthly precipitation totals for calendar year 2021 are presented in Table 2. Monthly recorded rainfall on-site ranged from 1.19 inches in October 2021 to a season high of 7.09 inches in June 2021. The average monthly rainfall amounts for Cofield, North Carolina (according to precipitation data from the USA.com website, www.USA.com), are presented on Table 2 and depicted on Figure 9. The amount of rainfall at the site in 2021 was approximately 3.6 inches less than the average rainfall reported for Cofield, North Carolina.
