On April 1, 2014, the Division of Water Resources (DWR) received a Riparian Buffer Authorization application dated April
2, 2014 for the above referenced project near Archer Lodge in Johnston County, NC The DWR has determined that the
application requires additional information as discussed below in order to process your application to impact protected
streams and riparian buffers on the subject property Therefore, unless we receive the additional information requested
below, we will have to move toward denial of your application as required by 15A NCAC 2H 0506 and will place this
project on hold as incomplete until we receive this additional information. Please provide the following information so
that we may continue to review your project.
Additional Information Requested:
The PCN Application did not include an agent authorization form Please send a completed form (include owner
and agent information, owner and agent signatures, and date) to this office at the address listed below I've
attached an example form that you may use if you wish. For time savings, you may email a scanned copy of the
signed document, but please also send by mail an original signed copy for our official file.
This correspondence only addresses the initial application review and does not authorize any impacts to wetlands,
waters or protected buffers. Please be aware that any impacts requested within your application are not authorized (at
