SPENCE JEANNIE YOUNG February 16, 2022 Robert Tankard, Assistant Regional Supervisor Water Quality Regional Operations Section North Carolina Department of Environmental Quality Division of Water Resources Washington Regional Office 943 Washington Square Mall Washington, NC 27889 SUBJECT: NOTICE OF VIOLATION & INTENT TO ACCESS CIVIL PENALTY TRACKING NUMBER: NOV-2022-DV-0040 Sanitary Sewer Overflows- December 2021 Collection System Permit No. WQCS00082 Elizabeth City Collection System Pasquotank County Incident Number: 202102263 On 12/14/2022 the City of Elizabeth City experienced a Sanitary Sewer Overflow (SSO) located at 115 Willow Way. A visual Estimation of 800 gallons reached surface water through the nearby storm drain. The cause of the spill was due to bypass pump failure at Hasting Lane lift station. During bypass operations the pump housing collected debris, and rags around the impeller. Also the suction hose was changed out due to cracks within the line. These issues caused the pump to lose prime. Hasting Lane lift station is connected to our telemetry system. All alarms were operable during the spill. Multiple lines tie in and gravity feed to Hasting Lane lift station. The flow remains - steady throughout the day. Currently the station remains on bypass until we receive our ordered pump, and second pump is in the shop for repairs. With the station having such a high level of flow daily inspections, and frequent services of the bypass pump are performed.
