On March 28, 2014, the Division of Water Resources (DWR) received a Riparian Buffer Authorization application dated
March 25, 2014 for the above referenced project-at,107 Aberson Court in Cary, NC. The DWR has determined that the
application requires additional information as discussed below in order to process your application to impact'protected
riparian buffers on the subject property. Therefore, unless we receive three copies ofthe additional information
requested below, we will have to move toward denial of your application as required by 15A NCAC 2H .0506 and will
place this project on hold as incomplete until we receive this additional information. Please provide the following
information so that we may continue to review your project.

The PCN Application did not include anagent authorization form. Please complete the form (including owner
and,agent information, signatures, and date) and mail to this office at the address listed below. (For time
savings, you may also email a scanned copy of the signed document, but please send an original signed copy
for our offiaal'file,)
