We also have the responsibility to the public to safeguard these records and to carry out our day-to-day program obligations. Please read carefully the following before signing the form. 1: Due to the large public demand for file access, we request that you call at least a day in advance to schedule an appointment for file review so you can be accommodated. Appointments are scheduled between 9:00 a.m. and 3:00 p.m. Viewing time ends at 4:45 p.m. Anyone arriving without an appointment may view the files to the extent that time and staff supervision are available. 2. You. must specify files you want to review by facility name or incident number, as appropriate. The number of files that you may review at one appointment will be limited to five. 3. You may make copies of a file when the copier is not in use by the staff and if time permits. There is no charge for 25 or less copies.: cost per copy after the initial 25 is 2.5 per copy Payment is to be made by check, money order, or cash in the administrative offices. 4. Files must be kept in the order you received them. Files may not be taken from the office. No briefcases, large totes, etc. are permitted in the file review area. To remove, alter, deface, mutilate, or destroy material in public files is a misdemeanor for which you can be fined up to $500.00. 5. In accordance with GS 25-3-512, a $25.00 processing fee will be charged and collected for checks on which payment has been refused. 6. The customer must present a photo ID, sign -in, and receive a visitor sticker prior to reviewing files. The report summarizes the findings of a recent inspection of the project's stormwater controls conducted on January 15, 2004 to determine compliance with Stormwater Permit Number SW8 960214 issued on May 8, 1996. A summary of the findings and comments are noted under Section 4 of the report. Please inform this Office in writing before February 16, 2004 of the specific actions that will be undertaken and the time frame required to correct the deficiencies. Failure to provide the requested information, when required, may initiate enforcement action. Compliance Issues: Y N ❑ ❑ The drainage area (including offsite) and built -upon area to the permitted system are per the approved plan. No unpermitted offsite drainage enters the system. ❑ ❑ All the built -upon area associated with the project is graded such that the runoff drains to the system. ❑ ❑ For ponds/infiltration, the outlet/bypass structure .relative elevations are per the approved plan. (Must be verified via as -built survey if in question). ❑ ❑ The inlets and outlet structure are located per the approved plans and do not cause short-circuiting. ❑ ❑ A trash rack is provided over the outlet weir and orifice. ❑ X All slopes are grassed with permanent vegetation and are not eroding. ❑ ❑ Vegetated slopes are no steeper than 3:1. ❑ ❑ The approximate amount of permitted surface area has been provided. (Must be verified via as -built survey or designer's certification.) ❑ ❑ Required drawdown devices are correctly sized per the approved plans. Nutnher 2 has some small erosion spats along its banks and slopes. These spP,t , are currently small and' not that bad, however, they will eventually become larger, and larger. These need to be filled with dirt and stabilized with grass or :nulch ASAP. Inspection Date: September 14, 2000 Weather Conditions: Clear and Warm 3. Compliance Issues: a. Both ponds #1 and #2 have recently been cleaned/dredged out. I was informed by Mr. Steve Sulkey, Century 21 Property Management, that the work was started . on September 2, 2000. The problem is that there were no trash racks for either pond. 4. b. Pond #2, located in the back, was completely stripped -of vegetation and hydro -seeded. The problem lies in the present season, Fall, the grass that is sown will have a hard time stabilizing the banks and slopes of the pond before Winter arrives: Another temporary type of vegetation may need to be planted. c. On January 27, 1998 an Compliance Inspection letter from Ms. Linda Lewis was mailed requesting that an Engineer's Certification be performed and submitted. To this date a certification has not been submitted to this Office. Required Actions: a. Mr. Sulkey informed me that the trash racks were on order and should be installed soon. It is Eastwood Developers, LLC responsibility to see that the ponds remain in compliance with the permit. b. Again I spoke with Mr. Sulkey about sowing winter rye to help stabilize the slopes. It again will be Eastwood Developers responsibility to see that the work is done. c. Please have the enclosed 2 page Professional Engineer's Certification completed, stamped, signed and dated. Inspection Date: January 14, 1997 Weather Conditions: Cold, sunny 3. Comparison of Approved plans vs. As -Built: Compared well 4. Maintenance Problems: a. Pond 1 was full of straw, apparently used as a mulch for grass seed. b. The trash rack for Pond 1 was missing. C. The Engineer's Certification has not been submitted. 5. PE Certification Received? NO Deed Restrictions Received? N/A 6. Comments: a. Please regularly clean up the pond area of floating debris to lessen the chance of clogging. b. Please construct the trash rack according to the approved plans. C. Please submit the Engineer's Certification upon completion of the items above.
