In accordance with the provisions of Article 7, Chapter 87, North Carolina
General Statutes, and other applicable Laws, Rules and Regulations,
PERMISSION IS HEREBY GRANTED TO
SINGER FURNITURE DIVISION
FOR THE CONSTRUCTION OF A MONITOR WELL SYSTEM located in the Residuum System of
the Great Smoky Mountain Hydrogeologic Unit at Singer Furniture Division on
Bushnell Road in Swain County, in accordance with the application dated
January 22, 1986, and in conformity with specifications and supporting data,
all,of which are filed with the Department of Natural Resources and Community
Development and are considered a part of this Permit.
This Permit is for well construction only, and does not waive any provisions
or requirements of any other applicable laws or regulations.
Construction of a well under this Permit shall be in compliance with the
North Carolina Well Construction Regulations and Standards, and any other State
and local laws and regulations pertaining to well construction.
This Permit will be effective from the date of its issuance until July 22,
1986, and shall be subject to other specified conditions, limitations, or
exceptions as follows:
1. Each well shall have permanently affixed an identification plate
constructed of durable material and shall contain the following
information: drilling contractor's name and registration number;
date well completed; total depth of well; a warning that the well
is not for water supply and that the groundwater may contain
hazardous materials; and depth to screens.

2. The wells shall be constructed with well casing and well screens
that conform to the requirements of Section .0107 of the Criteria
On Standards Applicable To Water Supply And Certain Other Type
Wells (15 NCAC 2C .0100) .
3. The well screens shall be set at a depth where the anticipated high
water table would be below the top of the screen so that any floating
petroleum products could enter the monitor well.
4. A one to two foot bentonite seal shall be placed above the well screen
and neat or sand cement grout placed above the bentonite seal to land
surface.
5. Provisions shall be made on the well cover (manhole cover) to prevent
surface water with possible contaminants from entering the well pit
and contaminating the monitor well.
6. Well completion, well abandonment, and water quality reports are
to be submitted to the Division of Environmental Management's
Groundwater Section Central Office in Raleigh.
7. All wells shall be afforded reasonable protection against damage
during construction and use.
8. When the well has served its purpose-and is of no further use, it
shall be permanently abandoned according to the requirements of
Section .0113 of the Criteria On Standards Applicable To Water
Supply And Certain Other Type Wells (15 NCAC 2C) .

6. When the wells have served their purpose and are of no further use,
they shall be permanently abandoned according to the requirements of
Section .0113 of the "Criteria and Standards Applicable To Water
Supply and Certain Other Type Wells" (15 NCAC 2C) .
7. Well completion, well abandonment, and water quality reports are to be
submitted to the Division of Environmental Management's Groundwater
Section Central Office in Raleigh.
8. The well driller shall notify the Asheville Regional Office prior to
actual construction of the wells.

Dear Mr. Blevins:
In accordance with your application received January 5, 1989, we are
forwarding herewith Well Construction Permit No. 86-0074-WM-0008 dated
January 11, 1989, issued to Haywood County Oil Company for the construction
of five monitor wells at Gas n' Groceries on US Highway 441 in Swain
County.
If any parts, requirements, or limitations contained in this Permit are
unacceptable to you, you have the right to an adjudicatory hearing before a
hearing officer upon written demand to the Director within 30 days following
receipt of this Permit, identifying the specific issues to be contended.
Unless such demand is made, this Permit shall be final and binding.
This Permit will be effective from the date of its issuance until July
11, 1989, and shall be subject to the conditions and limitations as
specified therein. Attached are Well Construction Records to be completed
and signed by the drilling contractor. These forms should be submitted to
the Department upon completion of the wells.

In accordance with the provisions of Article 7, Chapter 87, North
Carolina General Statutes, and other applicable Laws, Rules and
Regulations,
PERMISSION IS HEREBY GRANTED TO
HAYWOOD COUNTY OIL COMPANY
FOR THE CONSTRUCTION OF five monitor wells for Groundwater Incident No.
3819 in the Residuum System of the Great Smoky Mountain Hydrogeologic Unit,
located at Gas n' Groceries on US Highway 441 in Swain County, in
accordance with the application dated December 15, 1988, and in conformity
with specifications and supporting data, all of which are filed with the
Department of Natural Resources and Community Development and are
considered a part of this Permit.
This Permit is for well construction only, and does not waive any
provisions or requirements of any other applicable laws or regulations.
Construction of a well under this Permit shall be in compliance with
the North Carolina Well Construction Regulations and Standards, and any
other State and local laws and regulations pertaining to well construction.
This Permit will be effective from the date of its issuance until July
11, 1989, and shall be subject to other specified conditions, limitations,
or exceptions as follows:

1. Written permission must be obtained from the property owner prior to
construction of the wells.
2. The well driller shall notify the Asheville Regional Office prior to
actual construction of the wells.
3. The well screens shall be set at a depth where the anticipated high
water table would be below the top of the screens so that any floating
petroleum products could enter the monitor wells and of sufficient
depth so there is water adequate for sampling during the seasonally
low water table.
4. The wells shall be constructed with well casing and well screens that
conform to requirements of the "Criteria and Standards Applicable to
Water Supply and Certain Other Type Wells" (15 NCAC 2C .0100).
5. Each well shall have permanently affixed an identification plate
constructed of durable material and shall contain the following
information: drilling contractor's name and registration number; date
well completed; total depth of well; a warning that the well is not
for water supply and that. the groundwater may contain hazardous
materials; and depth to screens.
6. All wells shall be afforded reasonable protection against damage
during construction and use.
7. Well completion, well abandonment, and water quality reports are to be
submitted to the Division of Environmental Management's Groundwater
Section Central Office in Raleigh.
8. Provisions shall be made on the well covers to prevent surface water
with possible contaminants from entering the well pits and
contaminating the monitor wells.
9. When the wells have served their purpose and are of no further use,
they shall be permanently abandoned according to the requirements of
Section .0113 of the "Criteria and Standards Applicable To Water
Supply and Certain Other Type Wells" (15 NCAC 2C).

In accordance with the provisions of Article 7, Chapter 87, North Carolina
General Statutes, and other applicable Laws, Rules and Regulations,
PERMISSION IS HEREBY GRANTED TO
JOE ALMOND (MIDTOWN SHELL)
FOR THE CONSTRUCTION OF three (3) monitor wells, or more as needed, in the
Residuum System of the Great Smoky Hydrogeologic System, located at Main Street,
Bryson City in Swain County, in accordance with the application dated April 22,
1991, and in conformity with specifications and supporting data, all of which
are filed with the Department of Environment, Health, and Natural Resources and
are considered a part of this Permit.
This Permit is for well construction only, and does not waive any
provisions or requirements of any other applicable laws or regulations.
Construction of a well under this Permit shall be in compliance with the
North Carolina Well Construction Regulations and Standards, and any other State
and local laws and regulations pertaining to well construction.
This Permit will be effective for the construction of monitor wells from
the date of its issuance until the site assessment is completed, and shall be
subject to other specified conditions, limitations, or exceptions as follows:
1. The wells shall be constructed with well casing and well screens that
conform to requirements of the "Criteria and Standards Applicable to
Water Supply and Certain Other Type Wells" (15A NCAC 2C .0100).

2. All wells shall be afforded reasonable protection against damage
during construction and use.
3. The well screens shall be set at a depth where the anticipated high
water table would be below the top of the screens so that any floating
petroleum products could enter the monitor wells and of sufficient
depth so there is water adequate for sampling during the seasonally
low water table.
4. Provisions shall be made on the well covers to prevent surface water
with possible contaminants from entering the well pits and
contaminating the monitor wells.
5. Each well shall have permanently affixed an identification plate
constructed of durable material and shall contain the following
information: drilling contractor's name and registration number; date
well completed; total depth of well; a warning that the well is not
for water supply and that the groundwater may contain hazardous
materials; and depth to screens.
6. Well completion, well abandonment, and water quality reports are to be
submitted to the Division of Environmental Management's Groundwater
Section Central Office in Raleigh.
7. When the wells have served their purpose and are of no further use,
they shall be permanently abandoned according to the requirements of
Section .0113 of the "Criteria and Standards Applicable To Water
Supply and Certain Other Type Wells" (15A NCAC 2C).
8. If additional monitor wells, not shown on the location diagram,
need to be constructed, a map showing the proposed location shall be
submitted to the Asheville Regional Office.

8. When the wells have served their purpose and are of no further use,
they shall be permanently abandoned according to the requirements of
Section .0113 of the "Criteria and Standards Applicable To Water
Supply and Certain Other Type Wells" (15A NCAC 2C).

need to be constructed, a map showing the proposed location shall be
submitted to the Asheville Regional Office.

County Administration for the construction of a monitor well at the subject location, in Swain

you have the right to an adjudicatory hearing before a hearing officer upon written demand to the
Director within 30 days following receipt of this Permit, identifying the specific issues to be
contended. Unless such demand is made, this Permit shall be final and binding.
This Permit will be effective for the construction of a monitor well from the date of its
issuance until the site assessment has been completed, and shall be subject to the conditions and

PERMISSION IS HEREBY GRANTED TO
SWAIN COUNTY ADMINISTRATION
FOR THE CONSTRUCTION OF a monitor well for Groundwater Incident No. 13058
in the Residuum Aquifer System of the Blue Ridge-Inner Piedmont Hydrogeologic Unit,
located at the Joyce Cranford property, School House Road, Bryson City, in Swain
County, in accordance with the application dated December 22, 1995, and in conformity
with specifications and supporting data, all of which are filed with the Department of
Environment, Health, and Natural Resources and are considered a part of this Permit.
This Permit is for well construction only, and does not waive any provisions or
requirements of any other applicable laws or regulations.
Construction of a well under this Permit shall be in compliance with the North
Carolina Well Construction Regulations and Standards, and any other State and local
laws and regulations pertaining to well construction.

This Permit will be effective for the construction of a monitor well from the
date of its issuance until the site assessment has been completed, and shall be subject
to other specified conditions, limitations, or exceptions as follows:
1. Written permission must be obtained from the property owner prior
to construction of the well.
2. The well driller shall notify the Asheville Regional Office prior to actual
construction of the well.
3. The well shall be constructed with well casing and well screen that
conform to requirements of the "Criteria and Standards Applicable to Water
Supply and Certain Other Type Wells" (15A NCAC 2C .0100).
4. The well shall be afforded reasonable protection against damage during
construction and use.
5. The well screen shall be set at a depth where the anticipated high water
table will be below the top of the screen so that any floating petroleum
products can enter the monitor well and of sufficient depth so-there is water
adequate for sampling during the seasonally low water table.
6. Provisions shall be made on the well cover to prevent surface water
with possible contaminants from entering the well pit and contaminating the
monitor well.
7. The well shall have permanently affixed an identification plate
constructed of durable material and shall contain the following
•information: drilling contractor's name and registration number; date well
completed; total depth of well; a warning that the well is not for water supply
and that the groundwater may contain hazardous materials; and depth to screen.
8. Well completion, well abandonment, and water quality reports are to be
submitted to the Division of Environmental Management's Groundwater
Section Central Office in Raleigh.

9. When the well has served its purpose and is of no further use, it shall be
permanently abandoned by the applicant according to the requirements of Section
.0113 of the "Criteria and Standards Applicable to Water Supply and Certain
Other Type Wells" (15A NCAC 2C).
10. Upon completion of the monitoring well, the property shall be properly
cleaned and repaired back to its original state.

monitoring well permits for the installation of two (2) permanent monitoring wells.
S&ME proposes to install monitoring wells MW-1 and MW-2 on the northern portion of
the 25.6-acre undeveloped property (Parcel 667313035219), currently owned by Mr.
Jamie L. Winchester.
The monitoring wells are being installed to investigate the potential migration of solvent
related compounds from the northern adjoining property formerly operated as Ginchner
Shelter Systems. Ginchner Shelter Systems was a former active incident under the
jurisdiction of the North Carolina Department of North Carolina Environment and
Natural Resources (NCDENR) Division of Waste Management(DWM) Inactive
Hazardous Site (IHS) Section

In accordance with the provisions of Article 7, Chapter 87, North Carolina General Statutes, and other
applicable Laws,Rules and Regulations.
PERMISSION IS HEREBY GRANTED TO
Mountain Energy Corporation
FOR THE CONSTRUCTION OF A MONITORING WELL SYSTEM consisting of one monitoring well owned by
Mountain Energy Corporation,416 Stone Haven Drive,Waynesville,NC 28786.The well will be located on
property owned by Southeast Bulk,LLC,located in Swain County(PIN#667306396075).This Permit is issued in
accordance with the application received on July 3,2017, in conformity with specifications and supporting data,all
of which are filed with the Department of Environmental Quality and are considered integral parts of this Permit.
This Permit is for well construction only, and does not waive any provision or requirement of any other applicable
law or regulation. Construction of any well under this Permit shall be in strict compliance with the North Carolina
Well Construction Regulations and Standards (15A NCAC 02C .0100), and other State and Local Laws and
regulations pertaining to well construction.
If any requirements or limitations specified in this Permit are unacceptable,you have a right to an adjudicatory hearing
upon written request within 30 days of receipt of this Permit. The request must be in the form of a written petition
conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative
Hearings,6714 Mail Service Center,Raleigh,North Carolina 27699-6714. Unless such a demand is made,this Permit
is final and binding.
This Permit will be effective for one year from the date of its issuance and shall be subject to other specified conditions,
limitations,or exceptions as follows:
1. Issuance of this Permit does not supersede any other agreement,permit, or requirement issued by
another agency.
2. The well(s)shall be located and constructed as shown on the attachments submitted as part of the
Permit application.
3. Each well shall have a Well Contractor Identification Plate in accordance with 15A NCAC 02C
.0108(0)where applicable.
4. The monitoring well shall be maintained in accordance with 15A NCAC 02C.0108.
5. A Well Construction Record (GW-1) for each well shall be submitted to the Division of Water
Resource's Information Processing Unit within 30 days of the well completion in accordance with
15A NCAC 02C.0114.
6. When the well is discontinued or abandoned,it shall be abandoned in accordance with 15A NCAC
02C .0113 and a well abandonment record (GW-30) shall be submitted to the Division of Water
Resource's Information Processing Unit and the property owner within 30 days of the well
abandonment in accordance with 15A NCAC 2C.0114.
