Subject: Request for a NW 3 Permit and DWQ 401 for the replacement of a culvert structure in Mill
Swamp on US 13 in Gates County. WBS # 17BP.1.R.54
The North Carolina Department of Transportation is requesting the issuance of a NW 3 permit
and corresponding 401 certification for the replacement of a culvert structure on US 13 in Gates County.
The culvert is located on US 13 approximately 0.27 mile south of the intersection with SR 1300/ Paige
Riddick Road. The culvert is located in Mill Swamp, which is classified by DWR as class "C; NSW'
waters. The existing culvert structure is 3 @ 96" diameter X 60' long. These culverts will be removed using
NCDOT's guidelines for culvert removal. The proposed aluminum arch culvert will be placed on its
existing alignment as shown in the permit drawings. The new culvert structure is 34'2" X 11'5" X 58'6"
with aluminum headwalls located at the outlet and inlet sections of the culvert. The new culvert invert will
be placed more than 1 foot below the surveyed stream bed. Jurisdictional wetland and surface water
impacts have been minimized on this project. There may be some minor disturbance to vegetation during
sheet piling installation, but there will be no grubbing or permanent impacts. This project will require the
closure of US 13 for approximately 2 weeks. This temporary closure will be coordinated with the local
county officials and an offsite detour will be established.

application:
Project area consist of US 13 with the waterway of Mill Swamp passing under the roadway through 3@ 96 "X60' metal
culverts. The adjacent property use is a mix of agricultural, rural residential, and forested area.

3d. Explain the purpose of the proposed project:
Replace deteriorating metal culverts and maintain a stabile road way along US 13 for the safety of the traveling public.
3e. Describe the overall project in detail, including the type of equipment to be used:
The existing 3 - 60' long x 96 "dia. metal pipes will be entirely removed using NCDOT guidelines for pipe removal. A new arch
culvert pipe measuring 34'2" X 11'5" X 58'6" with headwalls will be placed as shown in the provided permit drawings. The new
arch pipe will be buried 1 foot below the surveyed stream bed. Once the pipe is in place and back filled a new roadway
surface and grassed shoulder will be established. The headwall tie in points will likely need some rip -rap stabilization as
shown on the permit drawings. Some temporary disturbance to the vegetated banks will likely occur from the dewatering
process and headwall installation. These banks will be stabilized following NCDOT erosion control bmp's. There are no
permanent impacts resulting from this maintenance project. This work will be accomplished using a track excavator, backhoe,
and dump trucks.

NCDOT will follow best management practices for soil and erosion control in compliance with N.C. Sediment and
