I’ve been able to split up the PDF into 12 PDF files that are under 10MB each. It seems I also was mistaken aboutthe required submittal documents for the permit mod. through the online portal. I have gone back and utilizedthe Word version of the PCN form and included that information in the updated submittal. Please expect aseries of 12 e-mails from me, which will contain the parts of the PCN PDF. Please let me know if you do notreceive one or more of them and I will attempt to re-send. I see the Permit Mod narrative and some impact maps, but I don’t see the PCN on the DWR side.

Hi Lyle, I apologize for the confusion on this. That PCN was one that was submitted due to a glitch in the system. I wasunable to submit via that route, and ended up submitting through the alternate web address for aModification/New Project with Existing ID. I submitted the correct application on 1/18/2022. I attempted to attach it, but it was too large for e-mail. If youdid not receive the correct application on submitted on 1/18, please let me know the best way to get it to you. Ihave access to a SharePoint account from which it could be downloaded, but if I recall correctly you are notallowed to click on the links. The file size is around 40MB. I am seeking an authorization under NWP 58 for the exact same NWP 12 authorized impacts, minus one streamimpact and one wetland impact per Tables 2 and 3 in the corrected application. Mr. Cooper,
I have reviewed the above referenced project and need the following comments/questions
addressed to evaluate the proposed activities.
1. It appears that there was an issue when the application was submitted. There are no listed
stream impacts on the PCN; however, the box for stream impacts was checked. Are stream
impacts being proposed? If so, could you please provide me a table for the proposed stream
impacts and plans showing the proposed activities. I have included the PCN for your review to
see if there are any additional omissions.
2. Additionally, the application was not signed. Could you please sign the last page of the
application. I have included the PCN for your review to see if there are any additional
omissions.

I also would like to schedule a call to discuss the project to ensure that I am understanding what
authorization is being requested. The changes in impacts are noted but based on our emails prior to
the submittal of the PCN I am unclear if you would like reauthorization of the activities previously
authorized under NWP 12.
