No, you filled it out correctly. The owner entity is Onslow County, but that entity must have an individual "Owner -
Owner" affiliation who is the legally responsible official for all permits tied to it. That person was Melissa Moore. Is it
okay to have that person be Randall now, given that it will be that way for all the other connected permits issued to
Onslow County?

I apologize, I must have filled a portion out incorrectly. Onslow County is the Owner. The contacts should all be Randall
Vanover. I think the portion on the permit summary that shows legally responsible for permit for NCG120023 where it
shows Owner Affiliation Melissa Moore is where my confusion came from. I apologize again and thank you for catching
this! Do you want me to recomplete anything?

Hi Kayla,
I've updated the outfalls and facility/permit contacts to Randall, but I paused before updating Owner Affiliation for this
permit, NCG120023. That's because the Owner entity here is Onslow County, and lots of other permits are tied to it. If I
change it to Randall, he will be the legally responsible person for this permit as well as the others I have highlighted
below. Is that okay?
If not, you all need to identify someone else with Onslow County that is appropriate to be affiliated with all of those.

Thank you so much! I have attached the form requested. Based on the Permit Summary Report it appears we also need
to update the following:
The Facility Contact Person should be — Randall Vanover, Solid Waste Director. The address and phone are still the same.
Email is Randall vanover@onslowcountync.gov
The Permit Contact Person should be the same information as the Facility Contact Person.
The Permit Billing Contact should be the same information as above.
Please let me know if we need to fill out anything else to get these portions updated. Thank you!
