We have 3 stormwater outfalls that discharge directly into the North Toe River. These are all stormwater, no
wastewater.
The outfalls are;

Thank You for getting back with me. Is there an online form to fill out or do I need to give you the
information? The information you have on the attched form will be the same for the other 2 outfalls, except
latitude and longitude. If I just need to send you the inforamtion let me know and I will forward it to

We received the attached letter regarding your issue with data entry into eDMR. I am sorry for your frustration with
this. Our supervisor Annette Lucas and Suzanne McCoy no longer work with us, and this is why they could not be
reached.
Currently, we only have one outfall listed in our permit system. We had collected outfall locations from permittees
through an on-line form some years ago, but we must not have received any entries for this one. For the outfalls that
you sample, we need to add that information so it can update the eDMR system.
For each additional outfall at your site, we will need the following information to add them:
Outfall Number (ex., 001)
Latitude/Longitude in degrees, minutes, seconds (like below)
Description (short description of what area it is or where it drains)
Whether it's Stormwater or Wastewater (including stormwater + wastewater)
Receiving waterbody name
Receiving waterbody stream index number (this on-line map is helpful for that, if it's a different one than this one
below)
