However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on March 16, 2022. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through F, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete. Please reference the subject application number when providing the requested information. Coyle February 14, 2022 Page 2 of 2 A. Residuals Land Application Program (FORM: RLAP 06-16) Application: 1. No comments. B. Operation and Maintenance Plan (O&M Plan): 1. No comments. C. Residuals Source Certification (FORM: RSC 06-16) Application: In the Summary for FORM: RSC 06-16 page, it is mentioned that the Tyson Foods River Valley Animal Foods — Harmony, NC facility and the AdvancePierre Foods — Claremont, NC facility sources were increasing their dry tonnage. This increase in permitted dry tonnage would be considered a major modification as defined in the footnote under Section B for the Instruction for FORM: RLAP 06-16. In accordance with section IV of the RLAP 06-16 form, a residuals source certification is required for all new, renewed, or modified residuals sources. An RSC form that meets the requirements in the Instructions for FORM: RSC 06-16 page shall be provided for each facility that is increasing the permitted dry tonnage. D. Land Application Site Certification (FORM: LASC 06-16) Form: Reciprocating setback waivers were provided between parcel 0100811 owned by Mathis Properties LLC (containing field NC-WK-MRF-12, among others) and parcel 0100020 owned by Blue Ridge Conservancy (containing the proposed field NC-WK-MRF-13, among others). The site map submitted in this application for the newly proposed fields on the Blue Ridge Conservancy property seems to show that field NC-WK-MRF-12 does not have the 50-foot setback that is shown for that field in Figure 21 of the permit issued February 4, 2021. The LASC summary page does not list the existing field NC-WK-MRF-12 as being modified to include this land application area, so it is unclear if the addition of this area is being proposed in this application. If this application proposes to add the new land application area to NC-WK-MRF-12 due to this setback waiver, the LASC summary page shall be resubmitted to include this field with its updated acreage along with an updated map to replace Figure 21 in the current permit. An abbreviated soils evaluation in accordance with Section B of the Instruction for FORM: LASC 06-16 shall also be submitted to verify that this section of the field is consistent with the soil series currently listed in the permit, and this shall be signed and sealed by a Licensed Soil Scientist (LSS). E. Landowner Agreement (FORM: LOA 06-16) Form: 1. No comments. F. County Board Notification (FORM: CBN 06-16): 1. In accordance with Section B of the Instructions for FORM: LASC 06-16, new land application sites shall provide a County Board Notification for each county containing a new land application site. A completed CBN that meets the requirements in the Instructions for FORM: CBN 06-16 shall be provided for Wilkes County.
