DANIEL SMITH Dlrecwr NORTH CAROLINA Environmental Quality February 11, 2022 The Honorable Luther Harvey Lewis, Mayor Town of Middlesex PO Box 69 Middlesex, NC 27557 Subject: Compliance Evaluation Inspection Middlesex Well No. 4 WTP NPDES Permit No. NC0087840 Nash County Mayor Lewis: On February 2, 2022, Mitch Hayes of the Raleigh Regional Office conducted the subject inspection with the assistance of William Lamm, Operator in Responsible Charge and James Liles, Public Works Director. Their help with the inspection was greatly appreciated. Below is a list of findings developed from the inspection and review of the permit: 1. The subject permit became effective September 1, 2018 and will expire April 30, 2023. 2. The 0.133 MGD PC-1 facility consists of the following units: raw water pumped from a single 185 gallons per minute well; liquid sodium hypochlorite disinfection system; two (2) each, circular 6 ft.-diameter greensand filters; potable water air -stripper system; one (1) flow meter for filter -backwash discharge; two (2) each, 8 ft. X 36 ft. wastewater drying beds and tablet dechiorinator. Discharge is to a roadway ditch, an UT to Turkey Creek classified as C, Nutrient Sensitive Waters within the Neuse River Basin. 3. At the time of inspection, all units were operational. There were some solids in basin # 1 and none in basin # 2. There was no discharge. Backwash occurs once a week. 4. Discharge Monitoring Reports (DMR's) for the review period January 2018 through December 2022 were reviewed for compliance with permit limit and monitoring requirements. The following were noted. Quarterly monitoring violation for Total Manganese for December 2018 and September 2019; Semi -Annual monitoring violation for Total Nitrogen and Total Phosphorus for December 2020. 5. DMR's from July, August, and September were compared to Environment 1, the contract laboratory, analytical results. There were no discrepancies.
