4. Project Description and History
4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:
EXISTING CONDITIONS STATEMENTS:
Project site 039 contains a section of Tandam Branch that passes through a box culvert beneath NC Highway 111. Project site 040 includes an approximately 1,000 LF section of Dark
Branch, starting at the outlet of two culverts passing under Dark Branch Road. Both sites are on prior converted agricultural land. These channels have experienced increased erosion,
scour, and related damages as a result of Hurricane Florence. Project site 043 is located alongside Sarecta Road and includes an unprotected roadside bank that has also experienced
severe erosion as a result of Hurricane Florence. No waters or wetlands of the US were seen in the project area.
PROJECT DESCRIPTION:
The project will include the repair and restoration of three (3) stream and drainage courses throughout Duplin County. These repair and restorations will include excess sediment removal,
bank grading, stream channel stabilization, the installation of in -stream structures and bank armor, and native vegetation planting. Grading shall be accomplished with an excavator and in -
stream structures shall be installed with an excavator with a hydraulic thumb.

4h. Explain the purpose of the proposed project: *
GPI was contracted by Duplin County to assist with 3 sites that were affected by Hurricane Florence. GPI has previously assisted the County with the initial Damage Survey Reports
(DSRs) which portions of this submittal utilizes information gleaned during that project. GPI has conducted topographic surveys and field reconnaissance work to collect data that will be
necessary to complete these projects. The sites are located throughout Duplin County, NC. Two project sites include Channel Bank Stabilization and Sediment Removal, while the third
project site is just a Bank Stabilization. The purpose of this project is to restore the drainage infrastructure to a condition which existed prior to the hurricane.
41. Describe the overall project in detail, including indirect impacts and the type of equipment to be used: *
The proposed stream section of Tandam Branch will utilize control structures to maintain grade, prevent unwanted erosion, and promote pool scour to increase biodiversity within the
stream. These structures will include 6 Log Sills that will be installed at the start of each pool section. They will allow the stream to hold grade during high -flow storm events. The banks of
the channel will be graded back at a stable slope and side slopes will be matted with fiber matting and staked with live stakes to restore bank integrity.
The proposed section of stream at Dark Branch will implement multiple Log Sill structures as a means for maintaining the designed grade while promoting beneficial pool scour. There are
16 proposed Log Sills throughout the Dark Branch reach that will be installed at the start of each pool section. They will be especially useful in maintaining grade at the upstream end, at
the outlet of the existing culverts. Culverts traditionally cause scour pools to form at both the upstream and downstream ends, and while they have desirable characteristics, they also tend
to increase in size over time. The Log Sill structures will prevent the culvert scour pool from moving downstream and causing an excessively large pool area. The channel banks will be
graded back to stable slopes and the side slopes will be matted with fiber matting and staked with live stakes to restore bank integrity.
To repair the failing bank along Sarecta Road, stabilization of the slope is necessary. Stabilization of the area will reduce sloughing and erosion of the roadside bank which is important to
protect the adjacent roadway and utilities. Due to the constraints of the project area and the risk to utility services, a structural approach was favored over a natural approach. The bank will
be graded back at a more stable slope and secured with flexible concrete armor. In addition to the concrete armoring, the site will be reseeded and vegetation will be able to take hold. The
project site is at minimal flood risk in Flood Zone
