An ACOE permit (SAW- 2009 - 2007 - 00387) was issued on 8/10/09 authorizing filling a small portion of
wetlands for the purposes of constructing a driveway to access the site The permit expired 8/10/11
prior to the construction of the driveway The current owner (Parks Family Leasing LLC) is currently
obtaining permits to begin building a residential home on the site This request is for the renewal of the
ACOE NW 18 permit and associated 401 wetlands permit Per discussion with Joanne Steenhuis, the
following is included in this package
• Application fee ($240, Ck #5759)
• Email letter of authorization
• (4) signed copies of the PCN application
• (4) copies of associated maps related to the site
