The permit was not renewed/lapsed when the new permit was released last year. So will it still be considered
"transferring the permit" even though it is not currently permitted? Should a new NOI be submitted?

The request indicates that there is a new owner. You need to fill out the Permit Name/Ownership change form. This
form you filled out is used to request a renewal COC held by the same permittee. To transfer the permit, we need the
current permit -holder and the new one to whom it's being transferred to sign.
I also see the address is supposed to be corrected to a different City. Is that a correction, or did the site move?
