Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the U.S. Environmental Protection Agency dated October 17, 2007 (or as subsequently amended.)
This permit authorizes the Iredell-Statesville Schools to discharge municipal wastewater from the
North Iredell High School facility to Patterson Creek, classified C waters in the Yadkin -Pee Dee River Basin. The
permit includes discharge limitations/or monitoring for flow, BOD5, total suspended solids, ammonia nitrogen,
total residual chlorine, fecal coliform, pH, temperature, total nitrogen and total phosphorus.
This final permit includes no major changes from the draft permit sent to you on January 27, 2009.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of
this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina
General Statutes, and filed with the office of Administrative Hearings, 6714 Mail Service Center, Raleigh, North
Carolina 27699-6714. Unless such a demand is made, this permit shall be final and binding.
Please take notice that this permit is not transferable. The Division may require modification revocation and
reissuance of the permit. This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of Water Quality or permits required by the Division of Land Resources, Coastal Area
Management Act, or any other Federal or Local govemmental permits may be required.

All previous NPDES Permits issued to this facility, whether for operation or discharge are
hereby revoked. As of this permit issuance, any previously issued permit bearing this
number is no longer effective. Therefore, the exclusive authority to operate and discharge
from this facility arises under the permit conditions, requirements, terms, and provisions
included herein.
Iredell - Statesville Schools is hereby authorized to:
1. Continue to operate an existing 0.0125 MGD wastewater treatment system with
the following components:
• Bar screen
• One (1) 8,000 gallon Flow equalization basin
• One (1) 2,500 Gallon Aeration basin with diffused air
• One (1) 1,600 Gallon Final clarifier with return sludge
♦ 6,000 Gallon Tertiary filters
♦ One (1) 2,000 Gallon Aerated sludge holding tank
• One (1) 2,000 Gallon Tablet chlorination and dechlorination
• 10 Gallon Flow meter
2. The facility is at the North Iredell High School located at 156 Raider Road in
Iredell County.
3. Discharge from said treatment works at the location specified on the attached
map into Patterson Creek, classified C waters in the Yadkin -Pee Dee River Basin.

On the basis of thorough staff review and applica-
tion of NC General Statute 143.215.1 and 15A NCAC
02H.0109 and other lawful standards and regula-
tions, the North Carolina Environmental Manage-
ment Commission proposes to issue a National
Pollutant Discharge Elimination System (NPDES)
wastewater discharge permit to the person(s) list-
ed below effective 45 days from the publish date
of this notice.
Written comments regarding the proposed permit
will be accepted until 30 days after the publish
date of this notice. All comments received prior to
that date are considered in the final determina-
tions regarding the proposed permit. The Director
of the NC Division of Water Quality may decide to
hold a public meeting for the proposed permit,
should the Division receive a significant degree of
public interest.
Copies of the draft permit and other supporting in-
formation on file used to determine conditions
present in the draft permit are available upon re-
quest and payment of the costs of reproduction.

Iredell-Statesville Schools (1147 Salisbury Road,
Statesville, NC 28625) has applied for renewal of
NPDES permit NC0037371 for the North Iredell
High School WWTP in Iredell County. This permit-
ted facility discharges .0125 MGD of treated
wastewater to Patterson Creek in the Yadkin -Pee
Dee River Basin. Currently BOD, fecal coliform
and total residual chlorine are water quality limit-
ed. This discharge may affect future allocations in
this portion of the Yadkin -Pee Dee River Basin.

Compliance Background
This is a permit renewal for a sanitary wastewater treatment system. The previous permit
included the new TRC limit of 28 ug/1 to take effect on 3/1/2006. The facility violated this
TRC Daily Maximum limit through Dec. 2006 even though dechlorination was installed. An
inspection on 1/26/07 determined that the facility did not purchase a meter capable of analyzing
effluent chlorine at the permit limit level until August 2006 despite the permit monitoring
requirement and notification from the Regional office. No TRC violations occurred in '07 and
`08, occasional weekly violations of BOD5 and Fecal Coliform occurred in Jan. through April
2008. Facility operator retired June '08 and Statesville Analytical was hired to operate the
facility for the school. Regional personnel stated concern about operational issues normally
associated with school systems when operations are suspended and restarted after holiday
breaks. Statesville Analytical contact Deena Myers stated that much of the equipment needed to
be fixed when they took over (blowers, timers) and she believes that following the
specifications listed by the design engineer have made significant improvements to the plant
operation. There have been no violations since Statesville Analytical has taken over.
Recommend that the permit be renewed.

PART IV - EVALUATION AND RECOMMENDATIONS
The facility has had compliance issues but most are related to operational activities. The staff
person associated with the non-compliance time frame has retired. There are operational issues
associated with the start up and holiday schedules normally associated with school only systems.
SOPs for observations and normal procedures associated with evaluating changes in biological
loading associated with facilities that haves Q$ flow conditions should be addressed as permit
conditions. It is recomme9d .that e,pCit be renewed.

Excess solids are measured in the clarifier. When the amount exceeds two feet, it is then
pumped to the sludge holding tank, which is a 3000 gallon concrete tank. There it is
dewatered with the supei"antant, pumped back into the flow equalization tank. When the
tank is approximately % full a certified septic hauler is called.
Lentz Septic Service, Inc. hauls and disposes of the sludge. Their permit number is NC
00147.
