The following is an abbreviated application form for those applying fora 401 ~'Vater
Quality Certification to impact FERC (Federal Energy Regulatory Commission)
regulated lake shore or bottom under the U.S. Army Corps of Engineers Permit
GP30. This Application cannot be used for wetland or stream impacts. Please
provide seven (7) copies of this application and supporting information as well as a
non-refundable application fee to the Division of Water Quality of $200.00 for
impacts of to lake bottom of less than 1 acre and $475.00 for impacts to lake bottom
of greater or equal to 1 acre.

the integrity of the shoreline. Baskets will be fronted by rip-rap to provide
fish habitat and to reduce wave reflection,

• Existing rip-rap will be pushed out into the water approximately 6' to clear room for laying a gravel bed for the
gabion baskets
• 3'x3'x6' Stainless Steel Galvanized Woven Gabion Baskets filled with small rip-rap will be linked in a continuous
gradual curve between Survey Marker # 1 and Survey Marker #3.
• The Gabions will be linked with stainless steel wire which is wound to link gabions together along the entire line at
all four corners. All exposed wire ends will be twisted for retention and tucked inward to prevent scraping and cuts.
• The leftmost and righmost gabion baskets will each be linked to a gabion basket at right angles or other angle through
custom modification of the gabion basket to tie into the slope (grade) and prevent erosion behind the baskets.
• The gabion baskets will rest on a bed of compacted railroad gravel adjacent to the current shoreline at normal pool
lake level. The width of the compacted railroad gravel bed will be between 3' and 6' and will be placed as close to
the current shoreline as is practical. Some space will be required for access to gabion corners etc. to properly secure
the baskets..
• Existing rip-rap will be pushed into the water prior to laying the foundation of railroad gravel and repositioned after
the gabions are completed to forma "toe" in front of the gabions.
• A sketch is provided below to illustrate the gabion basket positions as a line in green.
• The dock ramp will be replaced with a new arching ramp which spans over the gabions (clears the gabions). Final
staining of the dock ramp will be provided by the homeowner. Additional wooden steps will be added off the side of
the dock per the picture on the next page. Steps will include handrail but will match the existing wood on the dock.
• The existing stairs leading from the dock to the 420 line and up to the house will be repaired. Pressure treated 6x6
sleepers will be replaced with more permanent 6x6 stair structure composed of three sided box with deadman or
similar construction. Stairs will be extended to the 420 line to reduce erosion. Interior of pressure treated 6x6 box
will be composed of natural ground cover, gravel, pea gravel, or other loose aggregate to eliminate erosion and allow
soil absorption of rainfall and runoff.
• Vegetation adjacent to shoreline in the wooded areas to the left and right side of the property will be removed only to:
- Allow access to mechanized equipment if required
- Eliminate fallen or severly undercut root systems due to erosion
• The area will be temporarily stabilized with grass. Natural areas immediately above these temporary grassy areas will
be allowed to regrow.
• A subsequent plan may be submitted to reduce the total grassy area below the 420 line and provide for additional
erosion control. However, immediate action is required now to reduce further erosion to the shoreline.

• The existing stairs to the 420' line will be repaired with similar materials (pressure treated 6"x6"). Landscaping
timbers forming the edging on the stairs will be replaced with a more lasting structure of 6"x6"PT timber in a box
construction filled with natural ground cover or crushed stone (gravel, pea gravel, etc.) which absorbs rainfall and
runoff. Weed control cloth will be placed between the soil and the stair gravel underlayment to eliminate weeds.
• The top of the gabions will be level with the current height of the deck (to within several inches).
