These reports need either the signature of the facility manager or the signature of another (designated) senior company official. Sign the Stormwater Discharge Monitoring Report (DMR) form. o Insert the signed DMR into the SWPPP notebook at Appendix F. o Our previous email included directions for submitting the DMR to NCDEQ. Sign at the lower left of the BMP Checklist #6. o Insert the signed Checklist #6 into the SWPPP notebook at Appendix F. Insert the enclosed Laboratory Report and this letter into the SWPPP notebook at Appendix F. o Do not mail the lab report to NCDEQ — it is not requested. No parameters exceed a benchmark value in this sample. No further action is required. These enclosed reports satisfy the analytical monitoring requirements for the I' period of the 3rd year of the 2018-2021 General Permit NCG080000.
