For Bolivia Elem. School, we have a copy of the original topo. It is in the one -foot
contours. The flow is away from the buildings and goes across grassed surfaces.
The original topo also has the overall site boundary. We also changed our plans to
say that the property line is the site boundary.
Please call me or the architect when the permit is issued.

Bolivia Elementary School

Dear Mr. Macklin:
The Wilmington Regional Office received a Stormwater Management Permit Application for
Bolivia Elementary School on January 15, 2004. A preliminary revicw of that information has
determined that the application is not complete. The following information is needed to continue the
stormwater review:
1. The permit modification must include all the information on the previously permitted
plan. The new plans will replace the old plans. Please include all of the information in
Section VI of the application, including the following items:
a. Please provide proposed contour information to show that the proposed surfaces
will meet low -density requirements.
b. Please provide an overall site boundary.
Please note that this request for additional information is in response to a preliminary review.
The requested information should be received by this Office prior to March 12, 2004, or the application
will be returned as incomplete. The return of a project will necessitate resubmittal of all required items,
including the application fee.
If you need additional time to submit the information, please mail or fax your request for a time
extension to the Division at the address and fax number at the bottom of this letter. The request must
indicate the date by which you expect to submit the required information. The Division is allowed 90
days from the receipt of a completed application to issue the permit.
The construction of any impervious surfaces, other than a construction entrance under a!
approved Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject to
enforcement action pursuant to NCGS 143-215.6A.

Re: Bolivia School Expansion SW8 980733
Dear Linda,
The specifications/forms for the Bolivia School Expansion have been previously
submitted. This project consisted of creating a small addition to the existing school.
Laurie Munn sent us a copy of the old permit and relevant data. However, the data
provided was lacking in some details needed and after a meeting with Laurie, I was able
to complete the permit. The permit was then completed and returned the following day.
The contractor that bid the project planned and needs, to begin the work. I contacted
Laurie last week and learned that it may take some 60-90 days before it's reviewed, due
to a backlog of other pending projects. Since this is the last submittal after meeting with
Laurie, is there any way possible that she could complete this review? There are no storm
water pond, wetlands issues, or usual items. It's a matter of reviewing the figures on the
back of the form. Any assistance you could provide would be greatly appreciated.

The Division received an application for the subject projects on
December 22, 2003. The applications are incomplete and cannot be
accepted at this time. Please submit the following information to
accept the application for processing:
For modifications, the lower part of Section III.6 of the
application must include all of the previously approved BUA. I know
what the previous permit included, so I will just compare the new
numbers to the old numbers to see where the changes are.
Additionally, you must show the BUA on page 2 of the application -
calculations on separate pages are allowed only for multiple high
density systems. Since you have submitted the application on front
and back pages, I will return the applications for revision under
separate cover, or you can provide new originals with the requested
data.
Please complete Section VII on the South Brunswick application.
The checks have been made out to DEM, which was renamed in 1998 to
avoid conflicts with the Department of Emergency Management. We
have since been doing business as NCDENR- the North Carolina
Department of Environment and Natural Resources. Please have the
checks made out to NCDENR and make a note of this change for future
submittals.
Please submit the information by close of business Wednesday,
January 14, or the application will be returned.
