Dear \Ir. Saunders:
Division personnel have reviewed and approved your application for renewal of the subject permit. Accordingly, we are
forwarding the attached NPDES discharge permit. This permit is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection Agency
dated May 9, 1994 (or as subsequently amended.)
The final permit includes the following changes from the draft permit sent to you on May 12, 2004:
➢ The TRC limit added to the draft permit takes effect February 1, 2006. If you wish to install dechlorination
equipment, consult the enclosed guidance document.
➢ The instream monitoring for turbidity has been replaced with a turbidity effluent limit of 50 NTU, as you requested.
➢ The receiving stream name has been changed to "an unnamed tributary to Bashavia Creek". This changes the class
of the receiving stream to C.
➢ Your request for a higher TRC limit cannot be granted, given the existing discharge location. However, this permit
does include a reopener condition for the 11ZC limit. In order to receive a higher TRC limit, the City must either:
➢ designate the "dry ditch" as an effluent channel under the restrictions of 15A NCAC.0228, or
➢ pipe the effluent to the main channel of Bashavia Creek.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you, you
have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this letter. This request
must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the
office of Administrative Hearings, 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such a demand is made,
this permit shall be final and binding.
Please take notice that this permit is not transferable_ The Division may require modification (or revocation and
reissuance) of the permit. This permit does not affect the legal requirements to obtain other permits hat may be required by the
Division of Water Quality or permits required by the Division of Land Resources, Coastal .Area Management Act, or any other
Federal or Local governmental permits may be required. If you have any questions or need additional information, please contact

All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby revoked. As of
this permit issuance, any previously issued permit bearing this number is no longer effective. Therefore, the
exclusive authority to operate and discharge from this facility arises under the permit conditions, requirements,

A. (2) TOTAL RESIDUAL CHLORINE (TRC) REOPENER
This permit may be reopened for revision of the TRC limit if the Permittee completes one of the
following actions.
a. This permit may be reopened if the existing dry ditch between the outfall and Bashavia Creek
is designated as an effluent channel [per 15A NCAC 02B.0228]. To receive this designation,
the Permittee must submit documentation to the Division that shows the dry ditch:
> Is contained entirely on property owned (or otherwise controlled) by the Permittee
> Is not contain natural waters except when such waters occur in direct response to
rainfall events by overland runoff
➢ Is constructed or modified as to minimize the migration of fish int() said channel
b. This permit may be reopened if the Permittee constructs an effluent pipe from the existing
outfall to Bashavia Creek. This action would require an Authorization to Construct permit
from the Division
Any request to modify the permit must include complete documentation of the actions listed in either
a. or b. above, including ac revised topographic map.

Dear Mr. Weaver,
These comments are in reference to your letter dated May 12, 2004 and received by the
City of Winston-Salem on May 20, 2004 regarding the renewal of NPDES Permit #
NC0086762 for the Northwest Water Treatment Plant located at 2800 River Ridge Road,
Pfafftown, NC 27040. In regards to this draft permit the City of Winston-Salem
respectfully requests the following permit modifications:
1. The Northwest WTP is a new facility that is currently under construction. During the
site construction and grading, the topography of the site has changed and the
unnamed tributary that our discharge fed into is no longer a viable stream. Our
discharge is in effect the only flow feeding into this creek bed. This unnamed
tributary flows into the Yadkin River via Bashavia Creek. In light of this we request
that our discharge total residual chlorine limit be based on the flow of Bashavia Creek
and raised from 17 ug/L to 28 ug/L. In addition, the practical quantitation limit
(PQL) for total residual chlorine is 20 ug/L. Below this amount we cannot reliably
prepare standards to verify trace amounts of chlorine. The error associated with such
low level measurements may also be great enough to cause us to violate our permit,
when in fact we are not truly reading residual chlorine but interferences with the
method.
2. In addition, the City of Winston-Salem requests a turbidity limit of 50 NTU on this
discharge in lieu of the upstream and downstream turbidity sampling requirements
currently required. Due to the fact that we are now the headwater of this tributary,
this will simplify our monitoring requirements.

Creek.

Winston-Salem, NC 27102) has applied for
renewal of NPDES permit NC0086011 for the
Neilson WTP in Forsyth County. This permitted
facility discharges filter -backwash wastewater
to an innamed tributary, to Muddy Creek in the
Yadkin -Pee Dee River Basin. Currently total
residual chlorine is ater quality limited. this dis-
charge may affect future allocations in this por-
tion of Muddy Creek.
The City of Winston-Salem (P.O. Box 2511,
Winston-Salem, NC 27102) has applied for
renewal of NPDES permit NC0086762 for the
Northwest WTP in forsyth County. This permit-
ted facility discharges filter -backwash waste-
water to an unnamed tributary to the Yadkin -Pee
Dee Rive in the Yadkin -Pee Dee River Basin.
Currently total residual chlorine is water quality
limited. This discharge may affect future alloca-
tions in this portion of the Yadkin River.

Dear Mr. McIntyre,
On March 17, 2004 the Northwest Water Treatment Plant began an initial start-up to perform a
Site Acceptance Test for the plant control system. This required us to start processing water and
thus start discharging at our NPDES Discharge Point (Permit # NC0086762). Because this was
an initial start-up of the plant, we knew that the plant would be run for approximately 2 weeks
before being shut down in order to clean structures and disinfect in order to prepare the plant for
being put on line into our distribution system.
During this initial start-up period, the integrator for the project was still completing work with the
database that stores process data. During one of their functions, the company's programmer.
inadvertently deleted the database table that contained the flow data for our discharge for the
three days from March 17-19, 2004. This event was beyond our control and regrettably we
cannot retrieve this data. I have informed Rose Pruitt of this occurrence in addition to this letter.
In the future, our operators are going to manually record the flow data every two hours with their
process rounds so should the need arise we will have a second source for this flow data from our
discharge. At the time of this incident, the contractor's staff was operating the plant and the
City's staff was not onsite.

The City of Winston-Salem is requesting renewal of NPDES Permit #NC0086762 at our
Northwest Water Treatment Plant. As requested and required this letter describes the
Northwest Water Treatment Plant sludge management plan.
The Northwest Water Treatment Plant has two residuals lagoons located on the property.
Residual alum sludge from the facilities settling basins and backwash water is these
lagoons. The plant has residuals collectors in the basins that continually run and push
solids towards a residuals pit. On a timed frequency, automatic blow off valves open and
send the residuals to one of the plants two residuals lagoons. Once a residuals lagoon
becomes full, it is taken out of service and the solids are allowed to dry for several
months. Once dried, the solids are removed from the lagoon by a front loader and land
applied to an area the City of Winston-Salem has permitted on the plant site.

Dear Mr. McIntyre,
On Tuesday March 23, 2004 at 1:00 PM Mattie Mabe, Sam Tuttle and I met to discuss the
required upstream sampling point for the Northwest Water Treatment Plant (NPDES Permit #
NC0086762). The City of Winston-Salem requested this meeting because the tributary that was.
located at this point at the beginning of the Northwest Project, and which this sampling point was
based upon, no longer exists. The stream that was present -at that time was due to ground water
springs and the grading activities of the contractor have eliminated this flow.
Because we have lost an upstream sampling point for stream turbidity monitoring, we request that
the upstream turbidity -sampling requirement be waived. In our discussions with Rose, it was
decided that we would monitor turbidity from our discharge to insure that it remains low. We
will, collect the downstream sample as required.
