REQUEST FOR ADDITIONAL INFORMATION
Dear Mr. Holton:
On February 7, 2014, the Division of Water Resources (DWR) received your application dated
January 28, 2014 to impact 80 linear feet on stream and 6,460 square feet of Zone I and 1,920
square feet of Zone II of the Tar - Pamlico River Riparian Buffer. The DWR has determined that
your application was incomplete and /or provided inaccurate information as discussed below.
The DWR will require additional information in order to process your application to impact
protected isolated wetlands and /or streams on the subject property. Therefore, unless we
receive the additional information requested below, we will have to move toward denial of your
application as required by 15A NCAC 2H .1300 and 15A NCAC 213.0200 and will place this project
on hold as incomplete until we receive this additional information. Please provide the following
information so that we may continue to review your project.
Additional Information Requested:
1. It is required use of a temporary in stream check dams be requested in your Pre -
Construction Notification (PCN). It is also necessary to describe maintenance of check
dams while in stream and how check dams will be removed and area restored after
construction is completed.

2. It is necessary to illustrate areas where erosion control matting will be used on the subject
project.
3. PCN and plan drawings must show restoration plantings of the Riparian Buffers within
impact areas for Zone 1 in accordance with 15A NCAC 2B .0260, (9), (d), (e).
4. PCN form and /or plan drawings should indicate total number of linear feet of stream bank
improvements.
Please respond in writing to the DWR within four weeks of the date of this letter to with the
requested information, If we do not hear from you within four weeks, we will assume that you
no longer want to pursue this project and we will consider the project as withdrawn.
This letter only addresses the application review and does not authorize any impacts to
wetlands, waters or protected buffers. Please be aware that any impacts requested within your
application are not authorized (at this time) by the DWR. Please call Roberto Scheller of the

matter.
