SUBJECT: Application for CAMA Major Permit for Replacement of Onslow County Bridge #26 &
associated Utility Line Work and On -site Wetland Restoration Mitigation on
NC 50 approximately 1.7 miles South of SR 1104
Onslow County, WBS # 17BP.3.R.1
Dear Mr. Lane:
The North Carolina Department of Transportation (NCDOT) is requesting issuance of a CAMA Major
Permit to replace Onslow County Bridge #26 on NC 50. The proposal is to replace the existing 103.7 feet
long by approximate 26 feet wide 6 span concrete deck and timber joist and timber pile bridge with a 155
feet long by 33 feet wide 3 span 21 inch cored slab bridge on new alignment to the east of the existing
bridge as described in this package. The existing navigable vertical clearance would be maintained while
the horizontal navigable clearance would be significantly improved. The proposed bridge replacement
project would require utility work of relocating an aerial power line and an underground
telecommunications line. Onsite wetland restoration mitigation is proposed for this project as further
described herein.

4) Email correspondence with USACE Brad Shaver regarding preliminary discussions of proposed
mitigation.
5) Onsite Mitigation Plan dated February 18, 2014
6) Letter from the Department of the Navy regarding 50 year easement
7) NCDOT Right of Way Certification document
8) Notification letters to adjacent riparian property owner s and returned green cards from said owners
9) Supporting environmental documentation including:
a) Stream Form
b) Wetland and Upland Forms
c) Wetland Rating Form
d) Protected Species Assessment
e) Letter from the NC Wildlife Resources Commission
f) Architectural and Archaeological Clearances from NCDOT Cultural Resources staff
10) USGS topo and vicinity map (one map)
11) Permit Drawings (25 total permit drawings including the Impact Summary Table)
12) Bridge Survey Report (please note that the Bridge Survey Report is being included primarily for
information purposes and therefore has not been labelled as a permit drawing)
