Connie,
I'm looking at the NOI now, and the E&SC plan approval dates don't match up. The NOI says 9/21/2020, but the
approval email from the City is dated 12/28/2021. I know there was some back and forth with the City about erosion
control plans, but the final approval date must match.
I have to reject this NOI, which I'm very sorry about, and a new NOI will need to be submitted.
I also have to leave for the day, so I won't be able to do another review. Brooklyn will most likely do the final review on
Monday.

Hi Connie,
I wanted to give you a heads up that I'm about to reject the resubmission. It's the same issue as before. The NCG01 and
FRO form both say Clayton Properties Group Inc DBA Shugart Homes while the NOI certification form says Clayton
Properties Group Inc DBA Mungo Homes. If your FRO form is correct, you will need to change the NOI certification form
to match the information provided on the NCG01. If the NOI certification form is correct you will need to change the
information on your NCG01 (Tab B1 and E) and FRO form to match what is shown on the NOI certification form.

All — a follow up. I initially misunderstood that there was a change to the E&SC ID and not a correction. We cannot
update the information on the COC or record. We can only add information through a Modification form, so that is one
option.
The better option here, since it was so recently issued, might be to (1) terminate this COC and (2) submit a new
NOI. Because it was identified in 60 days or less, we can waive the fee for a new COC. We just need to be alerted
promptly when it's been submitted so we can communicate with staff to do that. The NOT reason can be "mistake" and
explain.

That is the right form to use, but please note you will not be able to revise any pre -populated information in the form or
revise the E&SC number on the original COC. We don't issue new COCs for modifications, but the documentation and
reason will be recorded in our repository. Please be clear in the explanation and include it in the notes so that it's in
the modification confirmation email once it's been approved.

Good morning,
I am emailing you regarding the NCG01 COC for this project. The Approved Grading Permit
EN2100279 is attached, along with the NCG01. The City of WS is telling us that we need to change
the Grading Permit number for this COC. Can you tell me how to address this issue? When I look at
the NCG01 that was uploaded, it appears to me to reference the correct Grading Permit....?
I have also included the COC.
