Subject: Permit Modification for Section 404 Nationwide Permit 13, 23 33 and Section
401 Water Quality Certification for the replacement of Bridge 46 over the Cape
Fear River on US 401 / NC 211 in Harnett County. Federal Aid Project number
BRSTP- 401(146). Debit $570 from 33490.1.1; TIP No B -4138
Enclosed please find the revised site plan for the temporary Rock Causeway for the above
referenced project.
At the Environmental Pre - Construction Meeting that took place on September 22, 2010,
the contractor, Dane Construction, Inc, had discussed the relocation of the Rock Causeway. The
attached site plan shows the proposed Rock Causeway directly under the existing Bridge 46. This
alignment will aid the contractor in the removal of the existing bridge and provide a safe means of
undertaking the demolition. We are requesting a Permit Modification for the Nationwide Permit
33 which addressed the Temporary Rock Causeway.

please accept this letter as our request to modify the permitted layout. The permit for this project
allows for a construction access and temporary placement of fill within 0.81 ac ofthe Cape Fear
River for two phase construction of a temporary amsiruc Lion causeway_ Attached is a copy of
our proposed modification for your iefereitce. We are requesting' that Phase 3 and 4 be allowed to
access the existing structure for demolition operations. Please consider our request and notify us
