Please update the agent authorization letter with both signatures. [15A NCAC 02H .0502(a)(11)]. See attached agent authorization letter. The revised stream impact table needs additional clarification. o S 1:The stream impact to install the culvert is shown as 188 LF to account for the loss of stream channel to be placed in a culvert. Thank you for updating this to reflect the loss of stream channel. o S2: The temporary dewatering impacts should only include the impacts to dewater the stream — such as where the impervious dike and pump is installed at the upstream location, along with the footprint to install the energy dissipation apron at the downstream location. Please update the impact table to reflect the linear feet of impact necessary for dewatering. See attached revised impacts table. o S3: The table shows 188 LF rip rap proposed- is this correct? I am assuming that the rip rap fill is for the headwall area upstream from the culvert inlet. Please update the impact table to reflect the linear feet of proposed rip rap below the ordinary high water line. See attached revised impact table. In order to show your project will be in compliance with Condition 17 in GC 4256, please provide a detail showing the proper installation (below the streambed elevation) of the proposed riprap pad at the culvert inlet. [15A NCAC 02H .0502(a) and (c)]. o I did not see a detail on sheet C3.0 showing how the rip rap pad at the culvert inlet will be installed below the streambed elevation. Is it on a different plan sheet? If not, please update the plan set to reflect this condition. See sheet C3.2 for detail.
