Dear Mr. Thomas,
We are requesting a Nationwide 14 permit for replacement of Bridge No. 69 with a triple barrel
box culvert on SR 1502 (Craver Road) over Reedy Creek (Class C) in Davidson County. I have
attached a preconstnxction notification application, construction plans, erosion control plans, a
stormwater management plan, and No Survey Required forms from NCDOT Cultural Resources
for Historic Architecture and Archeology. The project will permanently impact 112 linear feet
and temporarily impact 75 linear feet of Reedy Creek. Stream flow will be isolated into one
barrel, with 2' sills and floodplain benches constructed at the inlet and outlet of the overflow
barrels in order to prevent stream over widening and facilitate aquatic passage. The low flow
barrel will be buried 1' below the stream bed to ensure aquatic passage. The overflow basels
will be backfilled with class 1 rip rap to the sill elevation. The culvert site will be dewatered
during construction in accordance with NCDOT's Best Management Practices.
By copy of this letter, we also request 401 review and approval from the NC Division of Water
Resources with one copy of the application package provided to Amy Chapman, Transportation
Pernutting Unit Supervisor, and Dave Wanucha, DOT Project Coordinator. Please charge the
application fee of $570 to WBS project no. 17BP.9.R.18.
By copy of this letter, we also request Marla Chambers, NC Wildlife Resources Commission, to
comment directly to the Corps conceming the pemut request.

Project Description: Replace Bridge No. 69 over Reedy Creek on SRI502 (Craver Rd) in Davidson
Counry, North Carolina. The archaeological APEjor the project measures 600jt in [englh (300ftfrom
each bridge end point) by ISOft in width (75ft laterallyfrom each side oJthe SRI502 center-line). The
project constitutes, as currently proposed, an in place replacem.ent utilizing an o.jf-site detour during
construction activities. Some minar ditch-line impacts are scheduled to occur.
SUMMARY OF FINDINGS
The North Carolina Department of Transportation (NCDOT) reviewed the subject project and determined:

this project.

The pucpose of this letter is to notify you that the Ecosystem Enhancement Program (EEP) will pmvide the
compensatory stream mitigation for the subject project. Based on the information received from you on February 4,
2014, the impacts are located in CU 03040103 of the Yadkin River basin in the Central Piedmont (CP) Eco-Region,
and aze as follows:

N.C. Depar[ment of Envuonment and Natura] Resources' Ecosystem Enhancemen[ Program In-Lieu Fee Insh-ument
da[ed July 28, 201 Q If the above referenced impact amounts are revised, then this mirigation acceptance letter will
no longer be valid and a new mitigation acceptance letter will be required from EEP.
