at the subject facility. This chemical has been reviewed by the Division's Aquatic Toxicology
Unit.
After a review of the MSDS information you submitted, the Division has concluded that
use of CRP Blaze Off is acceptable at the stated concentrations. Please refer to the attached
memo from Melissa Rosebrock of the Division's Aquatic Toxicology Unit.

CP&L-Tillery Plant has submitted CRP Blaze Off cleaning agent for approval. A Material
Safety Data Sheet (MSDS) and letter describing its use have been reviewed and evaluated.
The proposed compound does not contain ingredients that are normally considered biocidal
and would thus not require individual review and approval using the standard biocide review process.
As with the use of many additives, effluent pH may be affected, and should be monitored. Following
current permitting practices, the use of this compound should be allowable. If there are questions

A(3). EFFLUENT LIMITATIONS AND MONITORING REQUIREMENTS - FINAL
FOR HYDROELECTRIC FACILITIES
Du ring the period beginning on the effective date of the permit and lasting until expiration, the Permittee is authorized to discharge waters from sumps &
drains (including powerhouse sumps & drains, wheel pit drains, head cover drains and gallery sumps & drains) and other miscellaneous wastewaters (from
sumps & drains associated with hydroelectric facilities)- from outfalls numbered serially beginning with 001 (specific outfall numbers shall be assigned by
the permittee). Such discharges shall be limited and monitored by the Permittee as specified below:

1: The tailrace shall be visually inspected for ail sheens when discharge occurs immediately after the lubrication of wicket gates (or other lubrication
operations which might result in the discharge of oil and grease) at the hydroelectric facility.
The p1-1 shall not be less than 6.0 standard units nor greater than 9.0 standard units.
y� Chemical wheel pit cleaning is permitted on a monthly basis. When cleaning of the wheel pits occurs, the tailrace shall be'inspected visually fax foam and
�11 oil. Cleaning shall be conducted only with pre -approved solvents. Should the Permittee wish to change cleaning solvents, a written request should be made
to the Division including the MSDS for the proposed solvent.
Mechanical cleaning operations which do not contribute any wastewater to the discharge are not limited by this permit. Non -discharging cleaning
operations may be conducted as often as necessary to ensure safety and proper facility operation.
The perrnittee shall obtain authorization from the Division prior to the use of any chemical additive in the discharge. The pernuttee shall notify the
Director in writing at least ninety (90) days prior to instituting use of any additonal additive in the discharge which may be toxic to aquatic life (other
than additives previously approved by ,the Division). Such notification shall include completion of Biocide Worksheet Form 101 (if applicable), a copy of
the MSDS for the additive and a map indicating the discharge point and receiving stream.
There shall be no chromium, zinc or copper added to the discharge except as pre -approved additives to biocidal compounds.
There shall be no .discharge of floating solids or visible foam (other than trace amounts) in the effluent.

The Division of Environmental Management has recently evaluated all existing individual permits for potential
coverage under general permits currently issued by the Division. 15A N.C.A.C. 2H .0127 allows the Division to
evaluate groups of permits having similar discharge activities for coverage under general permits and issue
coverage where the Division finds control of the discharges more appropriate in this manner. The Division has
determined that the subject discharge qualifies for such coverage. Therefore, the Division is hereby issuing the
subject Certificate of Coverage under the state-NPDES general permit no. NCG500000 which shall void NPDES
Permit NC0027529. This Certificate of Coverage is issued pursuant to the requirements of North Carolina and the
US Environmental Protection Agency Memorandum of Agreement dated December 6,1983 and as subsequently
amended.
If any parts, measurement frequencies or sampling requirements contained in this general permit are unacceptable to
you, you have the right to submit an individual permit application, associated processing fee and letter requesting
coverage under an individual permit. Unless such demand is made, this decision shall be final and binding. Please
take notice this Certificate. of Coverage is not transferable. Part II, E.4. addresses the requirements to be followed
in case of change of ownership or control of this discharge.
In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the
Permittee shall take immediate corrective action, including those as may be required by this Division, such as the
construction of additional or replacement wastewater treatment or disposal facilities. Construction of any
wastewater treatment facilities will require issuance of an Authorization to Construct from this Division.
Failure to abide by the requirements contained in this Certificate of Coverage and respective general permit may
subject the Permittee to an enforcement action by the Division of Environmental Management in accordance with
North Carolina General Statute 143-215.6A to 143-215.6C. Please note that the general permit does require
monitoring in accordance with federal law. The monitoring data is not required to be submitted to the Division
unless specifically requested, however, the permittee is required to maintain all records for a period ofat least
three (3) years.

The issuance of this Certificate of Coverage is an administrative action initiated by the Division of
Environmental Management and therefore, no fees are due at this time. In accordance with current rules, there
are no annual administrative and compliance monitoring fees for coverage under general permits. The only fee
you will be responsible for is a renewal fee at the time of renewal. The current permit expires July 31,1997.
This coverage will remain valid through the duration of the attached general permit. The Division will be
responsible for the reissuance of the general permit and at such time, you will be notified of the procedures to
follow to continue coverage under the reissued permit. Unless you fail to follow the procedures for continued

ATTACHMENT TO NPDES APPLICATION
TILLERY HYDROELECTRIC PLANT
ITEM 10
• Cooling Water consists of three sources: No. 4 generator air
coolers, air compressor cooling, and bearing cooling. The No.
4 generator air coolers discharge directly to the Pee Dee
River at a flow of .05-.06 MGD. Water from air compressor
cooling and bearing cooling is routed to the sump, which also
discharges at .05-.06 MGD.
Although the cooling water discharged is warmer than ambient
water temperature, the impact on the Pee Dee River is
negligible due to low discharge flow and temperature
differential. The combined cooling water flows, which are
less than one cubic foot per second (cfs), are insignificant
compared to the flow of the Pee Dee River, which averages 5400
cfs at the plant. Previous NPDES monitoring for temperature
has demonstrated that the heat input to the River does not
result in violation of water quality thermal standards.
• In addition to cooling water, water from dam leakage and floor
drains is also discharged to the Pee Dee River via the sump.

• The Tillery plant does not use any biocides or chemical
additives in the cooling water flows. The only substances
that the Tillery Plant could potentially add are 1) trace
amounts of metals from normal corrosion/erosion of
piping/equipment that comes into contact with water and 2) oil
and grease. Previous NPDES monitoring indicated that the oil
and grease concentration is normally very low. In the
unlikely event of an oil spill, oil and grease would be
collected in the sump. Should this occur, oil detectors in
the sump would activate alarms and disable the pump,
preventing discharge to the river. Any oil collected in the
sump would be removed with an oil skimmer and disposed of
properly. Additionally, the Tillery Plant has a Spill
Prevention Control and Countermeasure Plan which addresses
procedures for minimizing the risk of spillage.

ATTACHMENT TO NPDES APPLICATION
TILLERY HYDROELECTRIC PLANT
ITEM 10
• Cooling Water consists of three sources: No. 4 generator air
coolers, air compressor cooling, and bearing cooling. The No.
4 generator air coolers discharge directly to the Pee Dee
River at a flow of .05-.06 MGD. Water from air compressor
cooling and bearing cooling is routed to the sump, which also
discharges at .05-.06 MGD.
Although the cooling water discharged is warmer than ambient
water temperature, the impact on the Pee Dee River is
negligible due to low discharge flow and temperature
differential. The combined cooling water flows, which are
less than one cubic foot per second (cfs), are insignificant
compared to the flow of the Pee Dee River, which averages 5400
cfs at the plant. Previous NPDES monitoring for temperature
has demonstrated that the heat input to the River does not
result in violation of water quality thermal standards.
• In addition to cooling water, water from dam leakage and floor
drains is also discharged to the Pee Dee River via the sump.

• The Tillery plant does not use any biocides or chemical
additives in the cooling water flows. The only substances
that the Tillery Plant could potentially add are 1) trace
amounts of metals from normal corrosion/erosion of
piping/equipment that comes into contact with water and 2) oil
and grease. Previous NPDES monitoring indicated that the oil
and grease concentration is normally very low. In the
unlikely event of an oil spill, oil and grease would be
collected in the sump. Should this occur, oil detectors in
the sump would activate alarms and disable the pump,
preventing discharge to the river. Any oil collected in the
sump would be removed with an oil skimmer and disposed of
properly. Additionally, the Tillery Plant has a Spill
Prevention Control and Countermeasure Plan which addresses
procedures for minimizing the risk of spillage.

D. Biocide Condition
The permittee shall not use any biocides except those approved in conjunction with the permit
application. The permittee shall notify the Director in writing not later than ninety (90) days prior
to instituting use of any additional biocide used in cooling systems which may be toxic to aquatic
life other than those previously reported to the Division of Environmental Management. Such
notification shall include completion of Biocide Worksheet Form 101 and a map locating the
discharge point and receiving stream.

Dear Permittee:
The subject permit issued on 5/12/89 expires on 12/31/91. North Carolina
General Statute (NCGS) 143-215.1(c) requires that an application for permit renewal
be filed at least 180 days prior to the expiration date. As of the date of this
letter, the Division of Environmental Management had not received an application for
renewal.
If operation of a discharge or waste treatment facility is to occur after the
permit's expiration date, or if continuation of the permit is desired, it must not be
allowed to expire. A renewal request must be submitted no later than 180 days prior
to the permit's expiration date. Operation of the waste treatment works or
continuation of a discharge after the expiration date would constitute a violation of
NCGS 143-215.1 and could result in assessment of civil penalties of up to $10,000 per
day. If continuation of the permit is desired, failure to request renewal at least
180 days prior to expiration will result in a civil assessment of at least $300.00;
larger penalties may be assessed depending upon the delinquency of the request.
A renewal application shall consist of a letter requesting permit renewal along
with the appropriate completed and signed application form (copy attached), submitted
in triplicate, referenced in Title 15 of the North Carolina Administrative Code (15
NCAC) Subchapter 2H .0105(a). Primary industries listed in Appendix A of Title 40 of
the Code of Federal Regulations, Part 122 shall also submit a priority pollutant
analysis in accordance with Part 122.21. A processing fee must be submitted with the
application. In addition to penalties referenced above, a permit renewal request
received after the expiration date will be considered as a new application and will
require the higher application fee.

Attached is a copy of the fee schedule from 15 NCAC 2B.0105(b). The application
processing fee is based on the design or permitted flow, whichever is appropriate,
listed in the first five categories of facilities. Presently, no facility is allowed
to submit a fee for the general permits listed in the schedule since the Environmental
Protection Agency has not approved the State of North Carolina's general permit. If
the facility covered by this permit contains some type of treatment works, a narrative
description of the sludge management plan must be submitted with the application for
renewal.
The Environmental Management Commission adopted revised rules on October 1, 1990
(attached), requiring the payment of an annual fee for most permitted facilities. You
will be billed separately for that fee (if applicable), after your permit is_approved.
The letter requesting renewal, the completed Permit application, and appropriate

1986, concerning chlorine monitoring for cooling water discharges at the steam
electric generating plants. Your letter states that chlorine monitoring is required
at a plant whenever it is operating, regardless of whether it is using chlorine 9r
not. A review of the permit for Roxboro verifies this point. It is assumed that
all other permits are similar to the Roxboro permit.
The Division of Environmental Management has presumed that Carolina Power and
Light Company uses chlorine at its facilities on a regular basis. Your letter
indicates that this may not be the case. To clarify the matter, Carolina Power and
Light should conduct chlorine monitoring as specified by each permit during any
calendar month in which chlorine is used at each of the steam electric generating

The permittee shall obtain-uthorization from the Divislion of Environmental Management prior to utilizing any biocide
in the cooling water (S - - Part III of this Permit) .
The pH shall not be less than 6.0 standard units nor greater than 9.0 standard units
and shall be monitored monthly at. the effluent by grab samples.
There ��jll be no discharge of floating solids or visibh )am in other than trace amounts.

1. Continue to discharge non -contact cooling water, plant drain water, leakage
from dam joints, stormwater roof drains, and a transformer building drain
from a facility located at the Tillery Hydroelectric Plant at the end of
NCSR 1187 in Montgomery County (see Part III of this Permit) into the
Pee Dee River which is classified Class "WS-III & B" waters in the Yadkin -
Pee Dee River Basin.
