We did the required Waste Analysis on June 11, 2015 and mailed it as required. The consultant that sent
this in for us put cAsh in with the analysis report instead of a check or money order. The analysis did not
get recorded and the cash was removed. Please see the attached record as proof.
We deeply regret that this happened and for all future analysis reports we will be sure that the payment
is made in check or money order. Please accept our apologies for this matter and know that our farm
takes this responsibility very seriously.
Thank you for your assistance in this matter.
