ADDITIONAL INFORMATION
As required by 15A NCAC 02C .0105(f)(5), attach a scaled map of the site showing the locations of the following:

intersections, streams, or lakes within 500 feet of the proposed well or well system.
b. All existing wells, identified by type of use, within 500 feet of the proposed well or well system.

e. All sources of known or potential groundwater contamination (such as septic tank systems, pesticide, chemical or fuel
storage areas, animal feedlots as defined in G.S. 143-215.10B(5), landfills, or other waste disposal areas) within 500 feet
of the proposed well or well system.

I wanted to provide you with a copy of the report from the last time we were out at the Greenville City LF (attached
DP-5 report titled "REMEDIAL INVESTIGATION —ADDITIONAL WASTE DELINEATION, GROUNDWATER, and SOIL GAS
SAMPLING, GREENVILLE CITY LF").
As a reminder, Greenville City LF is a pre -regulatory landfill located off East Second Street between the Greenwood
Cemetery and the Tar River. This investigation, summarized in the report, helped tie up some loose ends from
previous field investigations. We did some surface debris mapping, which will help us when planning the remedial
action; determined the relationship between the location of the waste and the sanitary sewer line that bisects the
site; and also some additional sampling of groundwater and landfill gas. Methane was detected but does not
appear to be migrating off site. However, we are continuing to detect 1,4 Dioxane in the groundwater.
The below image is Figure 3 from the attached DP-5 report. If you look on the west side of the old landfill, you can
see the wells in which 1,4 Dioxane was detected in the groundwater (I highlighted the wells and the results in
yellow). The blue arrows show the groundwater flow direction, which is to the northwest in that area.

Because the contaminant appears to be moving off -site, I reached out to the adjacent property owners multiple
time, but I have not gotten a response from either. Because of that, I wanted to jump over those parcels to see if
the 1,4 Dioxane has moved across those parcels entirely.
The permission provided by the City in 2016 was not for this parcel, so I wanted to follow up to see if I could get
permission (from the City) to access the next property over (Parcel Number 38948).
Would that be OK with you? As part of the Greenville City LF investigation, I would direct our contractors to install
and sample 2-3 temporary groundwater monitoring wells and then remove/abandon them (per NCAC 02C well
construction standards).
