I see now, Michele.
There were two forms submitted for this Permit Name/Owner Change: one in 2018 and one in 2021. See attached.
The first one (6/18/2018) changed the company owner from Unimin to Covia Holdings Corporation, which was
done. That's what's in our database now:

The second one (7/21/2021) noted that the permit "prior to the change" AND "after the change" was issued to Covia
Solutions, Inc., which was actually not accurate —but close to what it was before. Our Admin noticed both these
sections were the same (including the legally responsible official listed) and probably assumed this form was only
submitted to change the Facility and Permit contacts only (which are different from 2018). It was not clear that this was
a request to change the owner entity name from Covia Holdings Corporation to Covia Solutions, Inc. (However, I see
several uploads were included about the company name change, and we have them on file).
So, I am guessing the company name change got missed, and our Admin mistakenly thought the 7/2021 submittal was
just a request to update facility and permit contacts for the current entity.

The note I have in my file from the consultant (Latitudes) we used for the name change says that it was submitted. But I
do not have confirmation that the transfer had been completed yet.

Do you know the status of the name change? If that has not been completed in our system yet, then it will hold up
eDMR registration.
The current permit summary report is attached. If it looks up to date, then the name change was completed, and you
can move ahead with submitting more current eDMR registration forms.
I expect this permit has not been registered (even in the owner is already in eDMR because of other wastewater
permits) — but only an eDMR Admin can access who has permissions. I will not be able to do that. I will forward the
question to our staff member helping. She'll either confirm this permit is not yet registered (or if you don't hear
anything, I'd assume it is not).
If eDMR registration steps need to happen for the new owner/contacts, please have them follow the Six Step Process
again so the registration information is accurate. It may take some time longer, but at least the most current
registration will be in the queue.

1. We are late on submitting our forms as we had a name change submitted in July and Step 1 was not updated. Is
this going to cause us an issue?
2. One of the questions on the forms is related to if we already have an account in a-DMR. Some on our list say

How to Submit Paper DMRs Before eDMR is Implemented

The Stormwater Program is planning for NPDES Stormwater Industrial permitted facilities to use
electronic Data Monitoring Reports by January 1, 2022. Until your facility begins using eDMR,
follow this NEW procedure for submitting eDMRs:
• Download the applicable DMR from the NPDES Stormwater Industrial Individual website or the NPDES
Stormwater General Permit website.
• Complete, sign, scan and upload the DMR to the NPDES Stormwater Permit Data Monitoring Report
(DMR) Upload form within 30 days of receiving sampling results.
• Mail the original, signed hard copy of the DMR to the appropriate DEMLR Regional Office.
• Do not send DMRs to Central Files in Raleigh! (This is a change from the previous procedure.)

I am reaching out as I just received a reply back from Suzanne McCoy stating that she is no longer the Stormwater
Administrative Assistant. A while back I had inquired about some corrections on our Marston facility in order to prepare

for the Step 1 & 2 process for the mining general stormwater permit. I know that the steps were due January 1, 2022 but
I am not sure where we are with the corrections.
