noticed drilling mud,at the'toe of slope and the�cul'vert extending,.undeineath Mt: Sinai
Road was completely inundated.
Location of the frac -out: Feature K =3, UT to New Hope Creek at Station 293+00.

Type of material: Bentonite clay and water slurry — standard HDD drilling mud.
Description of frac -out and efforts to protect sensitive areas: The drilling
operation was stopped immediately upon noticing the4rilling mud at the toe of slope.
The source ofthe drilling mud release turned out to be'through a seam in the concrete
reinforced culvert that extends,underneath Mt. ,Sinai. Tom Perez (S &ME Environmental
Inspector) was contacted immediately, and secondary containment,measures as shown,in
the attached pictures were installed to limit the migration of drilling,mud downstream and
to prevent any more from entering the stream. A back -hoe was used to dig a containment
pit as shown in Picture 1, and a pump was then installed in the containment pit in order to
recapture and recirculate'the drilling'mud- back`into the operation. Walter Cole (S &ME
Project Manager) reported incident to Chem Smith of the North Carolina Department of
Water Resources at 2:00 pm on Tuesday, February 4, 2014. Walter Cole and Tom Perez
performed an inspection together on February 5, 2014, assessing the containment
measures and documenting conditions.
Estimated size of frac =out (quantity and area): A fine layer of drilling,mud
impacted the stream for approximately 150 feet. The containment pit measured
approximately 4 feet X 4 feet X 4 feet. Due to the quick action of the contractor and
