MCCRORY GOVERNOR January 30, 2014 U.S. Army Corps of Engineers Regulatory Field Office Post Office Box 1000 Washington, N.C. 27889 -1000 Attention: Tracey Wheeler ANTHONY .J. TATA SECRETARY 20 140 1 39 Subject: Request for a NW 3 Permit and DWQ 401 Authorization for the replacement of Low Impact Bridge # 13 over Stumpy Point/ Lake Worth Canal on US 264 in Stumpy Point, Dare County. WBS # 17 B P. l . R.50 The North Carolina Department of Transportation is requesting the issuance of a NW 3 permit and corresponding DWQ 401 certification for the replacement of Low Impact Bridge # 13 in Stumpy Point, DareCounty. The bridge is located on US 264 approximately 0.28 miles south of the intersection with SR 1164. The bridge spans Stumpy Point Canal, which is within the Pasquotank River Basin. Stumpy Point Canal is classified by DWQ as class "C, SW" waters. Bridge # 13 is currently a four -span wood structure with a concrete deck supported by two interior bents with steel joist on timber piles. The existing structure is 100 ft. long by 26 ft., wide. This structure will be entirely removed using NCDOT's BMP's and guidelines for bridge removal. The proposed bridge will be replaced along its existing alignment. The new bridge will be a two span structure totaling, 125' in length by 33' wide. It will be comprised of a cast in place concrete deck and bents, supported by prestressed concrete piles and girders. Jurisdictional wetland and surface water impacts have been avoided and minimized. The resulting impacts are 0.008 acres of temporary hand clearing wetland impacts, and 0.089 acres of permanent wetland fill, primarily associated with the USFWS service road entrance relocation. There will also be 0.006 acres of permanent surface water fill associated with the road entrance relocation. The surface water impact ties into the canal that borders the service road on the south side. There will be no deck drains discharging directly from the bridge deck into surface waters of Stumpy Point Canal. Storm water has been routed to outlets located outside the canal channel on the north and south end of the project. The impact areas are located and detailed on the permit drawings. Traffic will be maintained on -site by the use of stage construction and temporary lane closures. This bridge is following the Low Impact Bridge Process established by FHWA, NCDOT and Regulatory Agencies. Included with this letter are the location map, permit drawings, and Low Impact Data Sheet. If you have questions please call me at (252) 482 -1876. The permit fee can be charged to WBS # 1713P.1.R.50 Sincerely, Jerry Jennings Division One Engineer /' Clay Willis w Division One Envir mental Officer Cc: Garcy Ward, DWQ Sonia Carrillo, DWQ TIP NO WBS Element No. 17BP.1.R.12 County Dare Bridge Number Bridge #13 Description Bridge is 100 ft. long X 26 ft. wide, the substructure consist on 3 interior bents and two end bents all supported by wood piles with concrete cap, the deck is supported by steel joist and the deck is concrete with concrete rails.
