Brian Lambe.

Good afternoon Mr. Thigpen,
My name is Brittany Carson and I am the General permits Coordinator for the Industrial Stormwater Program. We
received your application for coverage under NCG140000- Ready mixed concrete.
Your application does not list coordinates for outfalls for stormwater/wastewater discharges. At least one set of outfall
coordinates are required for the application. Please reply back with coordinates for said outfall(s). If you have any
questions, please let me know.
If we do not receive coordinates for your facility's outfalls within 14 days, your application will be returned and you will
be required to submit a new application.
