Salisbury Street Raleigh, North Carolina 27604 RE: NC Highway 150 Sewers DWQ # 12 -0764 Catawba County, North Carolina Dear Ms. Higgins: 7- Please see attached items pertaining to a revision to the PCN for the NC Highway 150 Sewer Service Project. The project has changed due to an extension to the west of the proposed sewer system. Installation of this additional linear sewer along Highway 150 will result in an additional ±40 L.F. of temporary stream impact, and approximately 0.02 acres of additional temporary impact to a wetland area. Upon completion of the utility line installation, the areas will be restored to their original conditions. These items are for review and approval of a modification to the 401 Permit for the subject project. ➢ Five (5) copies of the Revised PCN ➢ Five (5) copies of the PCN Attachment (property owners). ➢ Five (5) copies of an Exhibit showing the proposed utility extensions. ➢ Five (5) copies of the overall project map showing all stream and wetland crossings. ➢ Five (5) copies of the applicable Quad sheet showing project location. ➢ Five (5) copies of the Soil Survey Map. ➢ One (1) check #525081 in the amount of $240.00. The project has changed due to an extension to the west of the proposed sewer system. Installation of this additional linear sewer along Highway 150 will result in an additional ±40 L.F. of temporary stream impact, and approximately 0.02 acres of additional temporary impact to a wetland area. Upon completion of the utility line installation, the areas will be restored to their original conditions. These items are for review and approval of a modification to the 404 Permit for the subject project. ➢ One (1) copies of the Revised PCN ➢ One (1) copies of the PCN Attachment (property owners). ➢ One (1) copies of an Exhibit showing the proposed utility extensions. ➢ One (1) copies of the overall project map showing all stream and wetland crossings. ➢ One (1) copies of the applicable Quad sheet showing project location. ➢ One (1) copies of the Soil Survey Map.
