A. Application:
1. Please resubmit the application using the correct Division -approved form `Wastewater Irrigation
Systems — Renewal Form: WWIS-R-02-21', attached with the email for your reference. [15A
NCAC 02T .0105(b)]
B. Land Use or Lease Agreement:
1. Pursuant to 15A NCAC 02T .0504(0, please submit a land use or lease agreement pursuant to the
requirements in 15A NCAC 02L .0107(f), the Permittee shall ensure any landowner who is not the
Permittee and owns land within the compliance boundary shall execute and file with the Greene County
Register of Deeds an easement running with the land containing the following items:
- A notice of the permit and number or a description of the type of permit, and the name, address and
telephone number of the permitting agency; or a reference to a notice of the permit with book and page
number of its recordation if such notice is required to be filed by statute;
- Prohibits construction and operation of water supply wells within the compliance boundary; and
- Reserves the right of the Permittee or the State to enter the property within the compliance boundary for
purposes related to the permit.
C. Additional Documentation:
1. Back-up ORC Designation:
As per our records, a back-up ORC has not been designated for this facility. To designate a back-
up ORC, please submit a signed completed ORC designation form (attached with the email for your
reference) to the Water Pollution Control System Operators Certification Commission for approval.
